SUPPLEMENTS FOR ALL ADULTS
WASHINGTON, D.C., June 20, 2002 â Two Harvard researchers, Robert H. Fletcher, M.D., M.Sc., and Kathleen M.
Fairfield, M.D., P.H.D., of Harvard Medical School and the Harvard School of Public Health, have joined a growing list of
scientific experts who recognize the benefits of vitamins by stating in the June 19 issue of JAMA that, âwe recommend that
all adults take at least one multivitamin daily.â The researchers reviewed more than 30 years of English-language
articles about vitamins in relation to chronic diseases and published their findings in two companion articles.
In the scientific review article, the two physicians consider the evidence that sub optimal intakes of a number of vitamins
are associated with increased risk of chronic diseases including cardiovascular disease, cancer and osteoporosis.
In a clinical commentary, they note that âa large proportion of the general populationâ has less-than-optimal intakes of a
number of vitamins, exposing them to increased disease risk.
Annette Dickinson, Ph.D., vice president, scientific and regulatory affairs, Council for Responsible Nutrition (CRN), and the
author of The Benefits of Nutritional Supplements, a report to be published next week, states, âThere is no question that
the amount of scientific evidence in favor of consistent use of vitamins, particularly multivitamins, is formidable and
must be taken seriously, both by the medical community and by those who create public policy. As Drs. Fletcher and
Fairfield also point out, most people do not get an optimal amount of nutrients by diet alone. Supplements are a convenient
and affordable way to bridge the nutrition gap.â
With regard to B vitamins and cardiovascular disease, the Harvard researchers conclude: âUntil results of trials provide
more specific information on vitamin doses required to minimize homocysteine levels, recommending a daily multivitamin for
most adults may be the most prudent approach.â
On the question of vitamin E and heart disease, they recognize that some clinical trials have failed to find a beneficial effect
in people at high risk of heart disease or in patients with known coronary artery disease, but they suggest that vitamin E may
yet prove useful in the primary prevention of heart disease in the general population when taken for long periods of time.
Vitamin D supplementation is recommended at a level of 400 IU daily. This is the amount commonly included in multivitamin
supplements. In addition, the authors note, âThe addition of calcium may be required to realize the beneficial effects of
vitamin D in preventing fracture risk.â
In order to protect against having a baby with a neural tube defect, they suggest, âsupplementing all women who might
become pregnant with folateâ at a level of 800 micrograms per day. (The Food and Nutrition Board has recommended that
women capable of pregnancy should get 400 mcg of folic acid per day from supplements or fully fortified cereals, in addition
to the naturally-occurring folate in their regular diets). Dr. Dickinson adds, âClearly, this is more good news for those
consumers who already incorporate vitamins into their daily routine and should serve as a wake-up call for those who are
still just thinking about it.â

10. Yale Researcher Funded to Study Nutritional Biomarkers

Yale Epidemiologist, Susan Mayne, PhD, has focused her career on identifying the role of nutrition in cancer etiology.
This research area is challenging due to difficulties in assessing usual dietary intake in human studies. With the help of
two recent grants, she is a step closer to identifying non-invasive methods of nutritional evaluation. This Spring, Mayne
received $1 million in funding from the National Cancer Institute (NCI) to develop a non-invasive method to evaluate
nutritional status by measuring carotenoid levels in human skin, and another $60 thousand from the Breast Cancer
Alliance, Inc (BCA) to study the possible use of fatty acid profiles of nipple aspirate fluid as a biomarker of fat intake in
relation to the risk of breast cancer. Biomarkers allow researchers to establish objective predictors of nutrition for studies
of cancer and other diseases.
Because most nutritional epidemiology studies focus on people's recall of diet, the data can be suspect and is limited by
measurement error. Mayne's focus on carotenoid levels in the skin will give solid evidence of fruit and vegetable intake;
carotenoids are widely distributed in fruits and vegetables and absorbed following ingestion. This will allow further
research to be done evaluating the risk of cancer and its association with the amount of fruits and vegetables consumed,
using objective biomarkers. In collaboration with researchers at the University of Utah, Mayne and her colleagues will
work to develop and validate new technology for assessing the carotenoid levels of human tissue non-invasively through
the use of light.
Building on her twenty years of research on carotenoids, Mayne will be using a technique called Raman Resonance
Spectroscopy to assess carotenoid levels in human skin. Dr. Werner Gellermann, a physicist from the University of Utah,
developed the portable spectroscope. The spectroscope uses a light beam to excite the carotenoids in the skin and then
measures the vibrational energy produced, allowing for a tangible reading of carotenoid levels to be determined in less
than a minute. The study will compare the results of the carotenoid analysis done by spectroscopy with analysis of dietary
intake, blood carotenoid concentrations, and eventually carotenoid concentrations in human skin biopsies to validate the
use of the machine. Yale collaborators in this study include Drs. Brenda Cartmel, David Leffell, and Haiqun Lin.
The discovery of biomarkers of diet can better help researchers study the role of diet in chronic disease, and help
researchers interpret results of intervention studies aimed at improving diet. Also, the use of the raman spectroscopy
promises rapid feedback and has great potential for positive reinforcement of healthy eating, with the ability for easy
testing and re-testing for comparison of results. Ultimately, Mayne and her colleagues hope a derivation of the instrument
will be available for use by the general public for individual nutritional assessment.
With the assistance of the Breast Cancer Alliance, Inc., Mayne is also working with Dr. Brenda Cartmel to develop a non-
invasive predictor of breast cancer using nipple aspirate fluid and subsequently evaluating its content for fatty acids.
Researchers will use a device much like a breast pump to draw a tiny amount of fluid from healthy female study
participants and analyze its content using gas chromatography. Dietary data will also be collected in an effort to draw
conclusions between levels of fatty acids in the breast, blood, and diet. Because the nipple aspirate fluid can be obtained
non-invasively in women, there is potential for more widespread use. quot;We are excited to move ahead and test this on a
greater number of women,quot; Mayne explained. Mayne is hoping that the first year funding from the Breast Cancer Alliance
will allow her to generate the results she needs before moving ahead with etiologic studies linking fatty acid profiles in the
breast to the risk of breast cancer.
Because epidemiological research greatly depends on the quality and validity of available data, the development of
biomarkers significantly improves the credibility of the research and ultimately our understanding of diet and disease
relationships. Mayne's evaluation of both carotenoids and fatty acids at Yale Cancer Center brings significant promise into
two areas of cancer research and prevention.

BSCG is the only certification program in the United States that uses an IOC/WADA accredited
laboratory for all analyses. The detection limits used by BSCG are the lowest detection limits for any
