1. The authorization letter/SPA issued by the document owner, or
legal guardian shall specifically state that its’ purpose is to
secure civil registry documents from PSA. A general statement
as to the purpose will not be accepted.

registry document requested. However, the authorized
representative must be able to provide specific details required
in the application form. Otherwise, the request will not be

VI. Guidelines in cases where the COLB and COD requested belongs to a
Minor Child. Article 7 of the Presidential Decree No. 603 or the Child
and Youth Welfare Code shall be applied, to wit:
The record of birth and death of a minor child shall be kept
strictly confidential and no information relating thereto shall
be issued except on the request of any of the following:
1. His/Her parent or parents. In the absence of the parents,
the guardian or institution legally in-charge of him/her;
2. In case the child is a non-marital unacknowledged child,
only the mother can request for the COLB/COD.

requested is already dead. In case where the document owner of the
CRD is already dead, Section 17 of the DPA shall apply:

the rights of the data subject for, which he or she is an heir or
assignee at any time after the death of the data subject or
when the data subject is incapacitated or incapable of

the DPA shall be applied. The following guidelines must be followed:
a. SPA of the document owner must be presented prior to
the release of the document;
b. Should the document owner be deceased, the provisions
in Part III.C. of this Circular shall be applied when
applicable.
c. Should 13 (f) of the DPA be invoked or the legitimate
purpose for the request be for the establishment of legal
claims, a certification shall be issued either confirming or
denying the birth, marriage or death of the document
owner; and

VIII. Guidelines in cases where the requester is a law firm. In cases
where requests for CRDs originate from law firms. Section 12 and 13 of
the DPA shall be applied. The following guidelines must be followed:

order should be secured prior to the release of the
requested civil registry documents.
In all requests for CRDs, the principles of legitimate purpose,
proportionality, and transparency must always be upheld.

IX. Guidelines in cases where the requester is a government
agency
1. Requester is an agency with investigative powers and
subpoena powers.
The legitimate purpose including a determination of what the
extent of processing must be clearly stated in the letter
request. If none is stated, a certification should be issued
either confirming or denying the existence of birth, marriage or
death of the document owner. The request for CRD shall be
held in abeyance pending determination of the legitimate
purpose and proportionality of the requests from the
government agency.

2. Requester is from independent, central monetary authority and law
enforcement and regulatory agencies of their constitutionally and

The presentation of valid ID of the document owner in the issuance of civil registry
documents/certifications at the PSA CRS Outlets, LGU BREQS Partners and other PSA
authorized institutions/establishments must be strictly implemented.
Likewise, authorized representative is required to present his/her original and photocopy of
the ID and of the document owner aside from the presentation of authorization letter/SPA.
All IDs shall bear CLEAR PHOTO, FULL NAME IN PRINT and SIGNATURE and issued by official
authority.

workers cannot present the pro forma or template of Authorization, duly
signed by the DSWD Regional Director, where the social worker is situated.

There are cases wherein Independently Places (IP) children are

Processing of BREQS Transactions
Outlet Supervisors (OS) of the PSA and Team Leads (TL) and Unisys Managed
Services Corporation must strategize the distribution of all paid BREQS Batches
received by the Outlet to assure that these are also processed within the day of
receipt along with the walk-in applications.
In the event that the number of BREQS transactions are indeed too many and the
servicing PSA Regional/Provincial Outlet is unable to process all requests for valid
and acceptable reasons, the OM and/or the OS must immediately report the
matter to the Assistant National Statistician of the Civil Registration Service (ANS-
CRS) being the Project Director of the Civil Registry Information Technology Project
II (CRS-ITP2)

Validity of Birth, Marriage, and death Records issued by Philippine
Statistics Authority
Pursuant to RA 10625 or the Philippine Statistical Act of 2013 the PSA is
mandated to carry out, enforce, and administer civil registration functions in
the country. In line with this, PSA is the central repository of hard copies of
CRDs such as birth, marriage, and death records and issues printed copies in
SECPA upon request.
Pursuant to this mandate, all concerned agencies and individuals are
advised that CRDs printed in SECPA duly issued by the then National
Statistics Office (NSO), now PSA, have no expiration.

Philippine Identification (PhilID) Cards as the official proof of
identity for transactions with government and private entities
Under Republic Act No. 11055 or the Philippine Identification System Act, the
Phil ID shall serve as the official government-issued identification document
of cardholders in dealing with all national government agencies (NGAs),
local government units (LGUs), government-owned or controlled
corporations (GOCCs), government financial institutions (GFIs), State
Universities and Colleges (SUCs), and all private sector entities. As such, the
PhilID card shall be accepted as sufficient proof of identify, without the
need to present any identification documents.
