2. Chief complaints – swelling in the right
scrotum since the last 5 months
History of Presenting illness : Patient was apparently alright 5 months back when
he noticed a swelling in his left side of scrotum which was insidious in onset and
gradually progressive in nature .
No history of pain
No history of fever
No history of change in size of the swelling with posture
No history of painful micturition, burning micturition
No history of loss of appetite, loss of weight
No history of any abdominal mass or groin swelling
3. No history of abdominal mass or localized swelling
No history of trauma
No history of urethral discharge , evening rise of temperature , night sweats
No swelling over the left sided scrotum
Past history : no history of any chronic illnesses in the past
Personal history : no history any substance abuse in the past, no history of loss of
appetite, no history of bowel, bladder irregularities
4. General Physical Examination
Patient examined in adequate light in standing position with adequate exposure
PR 80 bpm , BP 120/80 mm hg
No Pallor icterus clubbing cyanosis lymphadenopathy edema,
Systemic examination : within normal limits
5. Local examination
Inspection : Right hemiscrotum appears to be normal
A globoid swelling of size roughly 6 x 5 x 5 cm in size roughly in the left
hemiscrotum
Surface of the swelling appears to be even
Skin over the swelling is smooth and normal in colour
No impulse seen on cough
Penis is in the midline with no obvious deviation
No scars , sinuses
Opposite testis appears to be normal
6. Palpation :
Inspectory findings of size and shape confirmed – globular swelling of 6 x 5 x 5 cm
seen confined to the left hemiscrotum with well defined margins
No local rise of temperature,no tenderness
Can get above the swelling
The swelling is cystic in consistency
Fluctuation test is positive
Translumination is positive
Testis is not palpable separately
7. Diagnosis
58 year old male with complaints of swelling in the left hemiscrotum since the
last 5 months, probably a case of hydrocele of the left testis.
