This study compared the effects of three phosphodiesterase-5 inhibitors - sildenafil, vardenafil, and tadalafil - on pulmonary and systemic hemodynamics in 60 patients with pulmonary arterial hypertension. All three drugs significantly reduced pulmonary artery pressure and resistance. However, the drugs differed in their time of maximum effect (40-45 minutes for vardenafil, 60 minutes for sildenafil, and 75-90 minutes for tadalafil) and their impact on oxygen levels (only sildenafil significantly improved oxygenation). In addition, only sildenafil and tadalafil, but not vardenafil, significantly reduced the ratio of pulmonary to systemic vascular resistance, indicating greater
