May I request your approval for the replacement of battery of the new issued service vehicle of
FSA Reneboy Delos Santos with plate number ZFR 522 amounting to P4,500.00 at Tyres
Master Trading, Santiago City . The said Battery was already worn out. The Isuzu crosswind was
service before by the resigned FSA Jose Aguinaldo.
Payment will be cash basis and budget coming from the imprest fund of Reneboy delos Santos .
