families who have low income or limited resources. Since
the 1960s, Medicaid has provided health care access for
millions—including children, individuals with disabilities, and
seniors. Today, Medicaid cuts threaten the availability of
quality care to our nation’s most vulnerable citizens—our
elderly and those living in poverty.

As of July 2012, thirteen states were moving to implement
Medicaid cuts by reducing benefits, paying providers less,
or limiting eligibility. Opponents assert that trimming
provider payments will cause providers to scale back
services or stop accepting new Medicaid patients. These
cuts come at a time when the federal government is
seeking to expand the program to cover 17 million
additional people.

Ironically, findings published in the New England Journal of
Medicine made the case for broadening Medicaid programs
just as states were enacting broad-scale cutbacks. The
study contrasted three states that expanded Medicaid
coverage with three that did not, and concluded that states
with expanded coverage experienced mortality declines.
This study suggests that Medicaid cutbacks threaten the
quality of life for at-risk citizens by limiting access to quality
health care.

Despite occasional ‘discovery pieces’ the biomedical literature in English has been almost entirely silent on the Cuban experience and US government policy temporarily forbade publication of articles from Cuba by US journals or their foreign subsidiaries. The unwillingness to take account of the Cuban experience, or to even view it as an alternative route through which some societies can move toward the universal goal of health promotion, represents an important oversight. In Florida, Medicaid expenditures have grown by 33% between 2012 and 2016. In 2016, the combined state and federal spending on Medicaid increased by 22%. Over these years, the federal government has provided between 55% and 60% of Medicaid funding. During the same time, Medicaid has growth to more than 75% of the Medicare expenditure and its share of national health expenditure has almost doubled. Following the passage of the Affordable Care Act, 2010, states were requested to expand their Medicaid program (Sutter, 2016). According to Ward (2020), the expansion of the Medicaid program has met significant resistant because of its presumed impact on state budgets. Florida is one of the states that have failed to expand its Medicaid plan under the Affordable Care Act. Some stakeholders argue that expanding the illegibility criteria is likely to have significant fiscal implications. Florida is among the states with the highest personal healthcare spending, ranking 5th nationwide. According to Buettgens (2018), the growing Medicaid expenditure are not necessarily driven by increased coverage, but also by changing medical needs, changes in demographics, and other factors affecting consumption of health care services. The debate on the cost implications of expanding Medicaid are likely to persist, but is evident that more Americans have health insurance. However, over time, it has not been clear to every American citizen as to which criteria should be used in determining the needy in the society. For instance, the issue of “spend down” periods has posed a lot of difficulties in determining the eligibility for Medicaid. A way great issue regarding the funding, expansion, and determination of eligibility for Medicaid includes the requirement by the Supreme Court in the United States for regions such as Massachusetts to contribute the six months’ excess income towards medical expenditure before the Medicaid coverage program begins in such regions. There are various healthcare disparities; however, the key areas of disparity I have chosen to identify and analyze include health insurance coverage, quality of care, and gender. As most people already know, health insurance is a type of insurance coverage that is designed to cover an insured person’s medical expenses (such as hospital, doctor, laboratory and pharmacy services). Although the number of uninsured Americans have decreased drastically since the passing of ACA, disparities in this area still exist. Some individuals’ annual incomes still are not enough to pay the low premiums that may be required of them to have access to health insurance coverage. Quality of care can be described as, “the degree to which health care services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge” (AHRQ, 2017). Although it is an unethical act displayed by healthcare professionals; in some areas, all patients are not treated equally with regards to quality of care. According to Dr. Ananya Mandal (2019), discrimination occurs when healthcare providers treat individuals from certain population groups differently to other population groups, whether this is done consciously or not. It is common for this to occur when providers have stereotyped impressions of specific racial or ethnic groups. Gender can play a major role in healthcare disparities among women, specifically, in some of the developed countries. Researchers have reported that determinants of gender differences, like welfare indicators (e.g., education and income), behavioral factors (e.g., smoking and drinking), and social factors (e.g., social support and socioeconomic status) have direct correlations with some of the existing disparities (Hassanzadeh, et al, 2017). Afghanistan is a country that still has a high rate of gender disparities among women, even though some improvements have occurred. In this country, the biggest disparities that I feel still exist are between women in rural versus urban areas, and those with some education, as opposed to those women with none; showing that as education of women increases, so does their health and that of their children because of the education and resources that they have. I think several economic policies have impacted these disparities and they include differences in income levels, education, and geographic location. Healthcare providers, on the other hand, are reimbursed based on a fee for service model, meaning the more services they provide, the more money they are reimbursed (Knickman & Kovner, 2015). In Pennsylvania, stakeholders include a steering committee, which is made up of hospitals, health care providers, consumers, foundations and academic institutions (“HIP”, 2019). This committee comes up with ways to improve population health and control health cost including Medicaid and Medicare. They developed a plan for heathcare delivery that will improve the quality of life for everyone, without limitations on income or background (“HIP”, 2019).
