• Shipping:
• Charges are “prepaid and add” with actual cost being charged at the time of invoicing. Quoted shipping
cost as provided in this authorization form are for estimation purposes only.
• Expedited shipping is available upon request and if requested additional charges will be applied to the
invoice.
• Drop shipping to multiple or alternative locations is available and if requested additional charges will be
applied to the invoice.
• Imprinted items may ship up to 10% over or under the amount ordered.
• Sales Tax: Charged on all Wisconsin orders unless an exemption certificate is provided at the time of order.
• Delivery: Unless otherwise noted, order delivery is approximately 2 – 3 weeks after the approval of the final art

• Art Work & Proofs:
• Client logos or other artwork are required in a vector format (.eps or .AI). If artwork cannot be provided in

• A paper or email art proof is required for all custom orders. This ensures accountability and prevention of
errors.
• An art proof will be available within <Insert # of days agreed upon with Dave Wied. Put this in bold black

• If multiple art proof be required additional charges will be applied to the invoice. Cost for art work is
