This document is a special verdict form from a jury trial in the case USA v. Nicodemo S. Scarfo, et al. regarding forfeiture under the Racketeer Influenced and Corrupt Organizations Act (RICO). The jury was asked to determine whether various assets, including vehicles, jewelry, bank accounts, stock certificates, and cash, were subject to forfeiture by each of the four defendants based on the RICO conspiracy charge in Count One.
