2. The knee is an intricate joint with numerous
tendinous, ligamentous, and meniscal
attachments, which make it particularly
vulnerable to complex injuries after trauma. In
the knee avulsion fractures can occur.
3. The prevalence of avulsion injuries continues to
increase as our population has been involved in sports
activities, sports and others. Although adolescents are
particularly vulnerable to this type of injury due to the
weakness of their apophyses, adults also may be
subject to avulsion fractures secondary to minor
trauma or major events, such as traffic accidents.
4. These fractures often have a subtle on
conventional radiography, advanced
imaging modalities, CT and MRI
in particular, are useful and can provide
valuable information to adequately define the
extent of damage.
5. The avulsion fractures specific to the knee joint,
including Segond and reverse Segond fractures;
avulsions of the anterior and posterior cruciate
ligaments; arcuate complex avulsion; iliotibial band
avulsion; avulsions of the biceps femoris,
semimembranosus, and quadriceps tendons;
Sinding-Larsen-Johansson syndrome; and Osgood-
Schlatter disease.
femoral, y los tendones semimembranoso, cuádriceps;
síndrome Sinding-Larsen-Johansson y la enfermedad de
Osgood-Schlatter.
6. this fracture involves cortical avulsion of the
tibial insertion of the middle third of the
lateral capsular ligament.
The mechanism of this injury is often the
result of internal rotation of the knee and
varus stress.
7. (a) Anteroposterior radiograph of the left knee shows an
elliptic bone fragment (arrow) arising from the lateral
tibial plateau. (b) Coronal fast spin-echo T1-weighted
MR image shows the nondisplaced avulsion fracture of
the lateral tibial plateau (arrow) with marrow edema
along the lateral tibial rim.
8. This fracture involves a fragment similar to that of
the Segond fracture except that it is located on the
opposite side of the knee, arising from the medial
aspect of the proximal tibia.
As is to be expected, the proposed mechanism of
this injury is also the opposite of the Segond
fracture, with external rotation and a valgus stress
applied to the knee being the most probable etiology.
9. (a) Anteroposterior radiograph of the right knee shows an elliptic
bone fragment (arrow) arising from the medial tibial plateau. (b) Coronal
fast spin-echo protondensity–weighted MR image shows the tiny bone
fragment (white arrow), which represents an avulsion of the deep
capsular component of the medial collateral ligament.
10. On conventional radiographs, avulsion fractures of
the ACL may be difficult to recognize. The appearance
of a tiny bone fragment in the intercondylar notch
with cortical irregularity of the adjacent tibial
eminence. MR imaging is useful to confirm that the
fragment does in fact arise from the tibia and that the
entire substance of the ACL is intact, as well as
assessing for associated injuries.
11. Anteroposterior (a) radiographs of the left knee show a bone fragment
in the intercondylar notch (arrow). The bone fragment arises from the
tibial insertion of the ACL.
Sagittal fast spin-echo proton-density–weighted (a) MR images show a
bone fragment avulsed from the tibia (arrow)
with an intact.
12. This injury of the posterior tibial articular
surface, a finding that is often best appreciated on
the lateral view. At MR imaging, a discrete bone
fragment is noted attached to an otherwise intact
PCL and separated from the remainder of the
tibia.
13. Sagittal fast spin-echo
proton-density–
weighted (a) and fat
suppressed T2-weighted
(b) MR images show an
avulsed bone fragment
(arrow) arising from the
posterior tibial plateau
with an intact PCL
(a) Anteroposterior
oblique radiograph of
the left knee shows a
bone fragment (arrow)
within the knee
joint.(b) Lateral
radiograph shows that
the avulsed fragment
(arrow) arises from the
posterior tibial plateau.
14. The arcuate complex provides posterolateral
stabilization of the knee and consists of the lateral
(fibular) collateral ligament; biceps femoris tendon;
popliteus muscle and tendon; popliteal meniscal and
popliteal fibular ligaments; oblique popliteal,
arcuate, and fabellofibular ligaments; and lateral
gastrocnemius muscle.
lateral.
15. On conventional radiographs, the “arcuate sign”
describes an avulsed bone fragment related to the
insertion of the arcuate complex at the fibular styloid
process.
MR imaging is useful to confirm the exact origin of
the fracture fragment, either by direct visualization or
by identification of marrow edema in the head of the
fibula and adjacent soft-tissue swelling.
16. (a) Anteroposterior
radiograph of the right knee
shows an elliptic bone
fragment (arrowhead) with its
long axis oriented
horizontally arising from the
fibularhead. (b) Lateral
radiograph shows the avulsed
fracture fragment (arrow).
The fragment is not as well
seen because it is
superimposed on the
posterior tibial plateau.
