2. In July 2004, eye surgeons from SINGAPORE NATIONAL EYE
CENTRE together with a dental team from NATIONAL DENTAL
CENTRE reported the first successful case of OSTEO ODONTO
KERATOPROSTHESIS better known as TOOTH IN EYE SURGERY.
The first successful case was that of a 19-year old boy from
THAILAND in 2004.

3. OOKP is a complex 2-stage
operation which aims to
restore vision to the most
severe cases of corneal
blindness and those patients
for whom all previous attempts
to restore vision had failed.

4. .
Corneal blindness means that
the cornea, which is the
transparent and thin outer
layer of eye, becomes cloudy
due to nutritional deficiency,
injury or diseases such as
trachoma, corneal ulcer,
degeneration, dystrophies,
etc. leading to loss of vision.
This blindness can be cured in
most cases by corneal
transplant. Corneal transplant
or grafting is performed on a
patient when a voluntary donor
pledges and donates his or her
eyes after death

5. The procedure involves
removal of canine tooth of
patient shaping and drilling it
to allow implantation of an
artificial plastic corneal
device (optical cylinder), and
ultimately implanting it back
into one eye a few months
later.

6. Stage 1
The surgery involves 5 separate
procedures:
1. Opening up one eye and removing
the entire inner surface of his
eyelids, his corneal surface, and
all scar tissue from his previous
eye injury.
2. Removing the inner mucosal lining
of the cheek and transplanting it
onto the new surface of the eye.

7. 3. Removing a canine tooth
and part of the
adjacent bone and
attached ligaments
from his jaw.
4. The surgeon fashions the lamina to be used in the
prosthesis by sawing longitudinally through the root
of the tooth to expose the dentine and the root
canal and then drilling a 3-mm to 4-mm hole to which
a plastic cylinder of suitable power is glued.

8. 5. Meanwhile, a subcutaneous pocket is created in the
patient’s cheek. The lamina-cylinder complex is
inserted in the pocket with antibiotic powder where
it will form connective tissue for suturing.

9. Stage 2 (about 4 months later)
This stage involves 2 separate procedures.
First, the cheek mucosal lining over the eye will be opened,
and a circular opening made in his cornea to receive the
implant. The inner contents of the eye will also be removed
at the same time.
Secondly, the living tooth-bone-
cylinder complex will be removed from
the cheek, and placed within the eye,
and the mucosal cheek lining will be
replaced over the implant.

10. At the end of the procedure, light can
now enter through the plastic cylinder,
and he will be able to see through this
cylinder with good vision.

Mumbai: Doctors at the famous Bombay Hospital and
Research Centre here performed a magic of sorts when they
managed to restore eyesight of a poor man from Uttar
Pradesh by performing the rare ‘tooth-in-eye’ surgery.
Sounds too technical and confusing – but it is true – a tooth
was implanted inside the eye of the man, who is nearing 60, to
restore his vision. And expert doctors of Taparia Institute of
Ophthalmology did all this. The surgery was performed free
of cost.
"Today I can see, I don't know, how to thank the doctors,
they are gods for me and my family. Howsoever, I thank them,
will be less," said Bakridi, the patient who hails from Basti in
Uttar Pradesh. The procedure over the past few months was
carried out by Dr Sonia Nankani and her team.
Dr Ashish Tiwari said that it was perhaps for the first time
that such an operation has been performed in Maharashtra, or
for that matter India. "We are collecting details from various
places to find out where such surgeries have been done," he
said.
The patient had been suffering problems in his vision for the
last 18 to 20 years. "There were ulcers in his eye and he lost
vision completely. He is a daily wage earner and hence the
livelihood his family too was affected," Dr Tiwari said and
pointed out that he underwent three cornea transplants
earlier – but all of them failed.www.indiandentalacademy.com
