This document is a youth membership application form for Cumann Lúthchleas Gael (Gaelic Athletic Association). It requests the applicant's name, address, date of birth, phone/email, and asks them to select a club and agree to abide by the rules of the GAA. The parent or guardian must also sign to consent to the application and certify that they have read and agree to the club's child protection policy for the year. Membership fees are listed for boys and girls of different ages.
