10. Mecanismo de reentrada El impulso que llega del NS se divide en las vías rápida y lenta. El mismo alcanzará al haz de His por la vía rápida, mientras que el que se propaga por la vía lenta, al llegar a este punto choca con el anterior, extinguiéndose ambos. Cuando aparece una extrasístole supraventricular lo suficientemente precoz, encontrará aún refractaria a la vía rápida, y se propagará en sentido descendente por la vía lenta. Al llegar al extremo ventricular del nodo AV, si la vía rápida está excitable en sentido retrógrado, el impulso se desplazará en forma ascendente por ésta, además de conducirse al ventrículo. Al llegar nuevamente al extremo craneal del circuito, volverá a encontrar excitable la vía lenta, descendiendo nuevamente por ésta y cerrando la reentrada. En este punto además despolarizará la aurícula en forma retrógrada, en forma casi simultánea al ventrículo.

La estructura de los nodos y el resto del sistema de conducción AV en el corazón humano es comparable a la de los animales normalmente utilizados en el laboratorio; no obstante, existen diferencias entre especies. En un extremo de la aurícula derecha se sitúa el nodo SA, que es la estructura responsable de generar el ritmo cardíaco. Posteriormente, el impulso viaja en el espesor de las paredes auriculares, a través de una intrincada pero precisa disposición espacial de las fibras auriculares de trabajo, hasta el extremo final de la aurícula. En este último extremo, las células transicionales del nodo AV están «abiertas» para recibir y retrasar el impulso antes de su transmisión al haz de His. Este haz atraviesa el plano fibroso de aislamiento AV y lleva el impulso a través de 2 ramas (derecha e izquierda de His) que se dirigen a cada uno de los ventrículos. Cada una de estas ramas está aislada mediante una envoltura conectiva de los miocitos ventriculares de trabajo. Esta disposición permite contactos entre los miocitos especializados y los de trabajo solamente en las ramificaciones distales de las ramas de His. De esta manera, el sistema de conducción AV, descrito en su mayor parte por Tawara2 hace casi 100 años, está estructurado para facilitar un orden en la transmisión del impulso cardíaco. Conocer la estructura y localización del tejido específico de conducción dentro del corazón podría proporcionar soluciones potenciales a las diversas perturbaciones que se producen del ritmo cardíaco.

BACKGROUND We recently reported an ECG algorithm for differential diagnosis of regular wide QRS complex tachycardias that was superior to the Brugada algorithm. OBJECTIVE The purpose of this study was to further simplify the algorithm by omitting the complicated morphologic criteria and restricting the analysis to lead aVR. METHODS In this study, 483 wide QRS complex tachycardias [351 ventricular tachycardias (VTs), 112 supraventricular tachycardias (SVTs), 20 preexcited tachycardias] from 313 patients with proven diagnoses were prospectively analyzed by two of the authors blinded to the diagnosis. Lead aVR was analyzed for (1) presence of an initial R wave, (2) width of an initial r or q wave 40 ms, (3) notching on the initial downstroke of a predominantly negative QRS complex, and (4) ventricular activation–velocity ratio (vi/vt), the vertical excursion (in millivolts) recorded during the initial (vi) and terminal (vt) 40 ms of the QRS complex. When any of criteria 1 to 3 was present, VT was diagnosed; when absent, the next criterion was analyzed. In step 4, vi/vt >1 suggested SVT, and vi/vt <1 suggested VT. RESULTS. The accuracy of the new aVR algorithm and our previous algorithm was superior to that of the Brugada algorithm ( P .002 and P .007, respectively). The aVR algorithm and our previous algorithm had greater sensitivity ( P .001 and P .001, respectively) and negative predictive value for diagnosing VT and greater specificity ( P .001 and P .001, respectively) and positive predictive value for diagnosing SVT compared with the Brugada criteria. CONCLUSION The simplified aVR algorithm classified wide QRS complex tachycardias with the same accuracy as standard criteria and our previous algorithm and was superior to the Brugada algorithm (Heart Rhythm 2008;5:89 –98)
