This is to confirm that the above-captioned mediation has been scheduled for Tuesday,
November 1, 2011 at 10:30am at my office, located at Interstate Plaza Executive Offices, 1499
W. Palmetto Park Road. First Floor, Boca Raton, Florida 33486. Although it is not mandatory,
you may feel free to fax or mail a mediation summary to my attention prior to the mediation.
This will also confirm your agreement that the mediation fee is $300.00 per hour with a two (2)
hour minimum fee. Full payment shall be made at the conclusion of the mediation via check
?t made payable to "Lance J. Wogalter. P.A.". There will be no fee for canceling a mediation, if a
written or fax notice of such cancellation is received more that four (4) full business days prior to
the scheduled mediation. Otherwise the canceling party shall be responsible for a one (1) hour
cancellation fee. Counsel should make arrangements with their clients for the payment of any
minimum fee, cancellation fee or balance due. inasmuch as we would look to counsel for the
parties to effect collection or make payment.
Thank you for scheduling the mediation with my firm and we hope that the matter can be
successfully resolved.
