1. PATIENT SAFETY IS EVERYONES RESPONSIBILITY!
Below are a few simple tips to improve safety and provide steps to lower your risk of drug errors.
Share your health history with your PCM team
Share up-to-date information about your care with everyone who’s treating you
Pay attention. If something doesn’t seem right, tell your PCM team
Be sure you understand all your instructions, ask questions if you do not understand
Take drugs exactly as prescribed; DO NOT share your drugs
To avoid possible drug interactions, tell your pharmacist/PCM team all drugs you are taking,
even over the counter meds/herbal supplements
Checkthat the pharmacy has your current allergies/drug intolerances and phone number
Find out how many refills you have~~remember prescriptions are good for one year
Don’t rely on memory; always double check the label and dose prior to taking meds
Keep unwanted meds in their original bottle. Use permanent marker to cross out your name
and prescription number on the label
