The teacher was nominated for the prestigious Presidential Award for Excellence in Mathematics and Science Teaching. As part of the application process, the teacher's AP Calculus BC class was videotaped and the teacher is required to post the video online. The teacher requests permission from parents/guardians to post the video by having them sign and return the permission slip. The award is highly competitive with only one teacher from each state selected every other year. The teacher hopes to win the award and represent their high school.

Dear Parents/Guardians of BSHS AP Calculus BC (Math 5HBC) students:
As a nominee for the Presidential Award for Excellence in Mathematics and Science Teaching
(PAEMST), I am required to make a video recording demonstrating my practice of teaching
Mathematics. My 1st period Math 5HBC class was recorded on Thursday 9/6/2012 for this purpose. As
a part of the application process, I am also required to post this video on-line. By signing this form, you
give me permission to do so.
The PAEMST is a very prestigious and competitive award. Every other year, only one High School
Mathematics teacher and only one High School Science teacher will be selected from each state. Those
selected for this honor will spend a week in Washington DC meeting with the President of the United
States and the President's Cabinet to discuss ways to improve the practice of teaching and learning
Science, Technology, Engineering and Mathematics (STEM) in our nation's High Schools.
I am humbled to have been nominated for this award. I would very much like to bring this distinction
home to BSHS! However, I need your help. May I ask you and your child to please sign and return this
