4. CONTEXTO I
Hay una necesidad de descartar o confirmar infarto agudo de
miocardio (IAM) de manera rápida y segura en pacientes con dolor
torácico agudo y consigo:
1. Iniciar rápida y oportunamente el manejo del IAM
2. Limitar el abuso de los recursos en salud en urgencias,
egresando los pacientes sin condiciones cardiovasculares de
urgencia.
Las guias1,2 actualmente recomiendan la medición de troponina
ultrasensible al momento del ingreso y a las 3 horas posteriores,
detectando niveles elevados y su posterior elevación o disminución.
Los estudios recientes ADAPT3 (2 horas) y APACE4 (1 hora) han
propuesto intervalos de tiempo menores a las 3 horas.

Los pilares del diagnóstico de IAM lo constituyen historia clínica, EKG
y troponina de alta sensibilidad.2,3
Actualmente una de las mayores debilidades de la prueba de
troponina cardiaca (cTn) es la dificultad para detectar niveles de cTn
bajos lo que conlleva a la toma seriada por 6 a 12 horas.3

11. OBJETIVO
• El objetivo consta de la validación externa de
la precisión diagnóstica del algoritmo de 0/1
horas con troponina T ultrasensible (hs-cTnT)
para la rápida confirmación (rule-in) y descarte
(rule-out) del IAM y con ello evaluar su
aplicación en el uso clínico de rutina.

Criterios de inclusión:
- Dolor torácico agudo (<6 hs) sugestivo de IAM.
- Que se haya tomado muestra hasta máximo 45 minutos después
del ingreso.
- Firma de consentimiento informado
Criterios de exclusión:
- Falla renal estadio V
- Trauma.
- Desfibrilación o cardioversión al ingreso.
- requerimiento de trombolisis
- POP <1 mes CABG.
- Dx IAM < 3 semanas.

independientemente utilizando toda la información disponible*
incluyendo troponina I (99th percentil 40 ng/L ó ∆ > 20ng/L).

30. DISCUSIÓN
• Estudio observacional aplicado en el escenario de urgencias
• En pacientes de baja probabilidad?
• Rol en paciente en TRR?
• Rol en pacientes con síntomas atípicos?
• Rol en pacientes con > 6 horas de presentación del dolor?
• Extrapolable a nuestra población?
• Se incluyen entre APACE y TRAPID-AMI mas de 3500 pacientes en total.
• Se debe evaluar el VPN analizando el biomarcador+EKG+clínica
• El algoritmo descarta IAM pero no angina inestable y otras condiciones
cardiacas.
• Tener en cuenta otras causas de elevación de troponinas.
• Los pacientes en la zona observación merecen abordaje cauteloso

myocarditis, takotsubo cardiomyopathy,acute heart failure, or tachyarrhythmias
most common other diagnoses in the rule-in zone weremyocarditis (4), unstable angina (4), takotsubocardiomyopathy (3), heart failure (3), arrhythmia(3), and symptoms of unknown origin (16).
Among the 285 patients classified to the observationalzone, 64 received an adjudicated diagnosis of acutemyocardial infarction, resulting in a prevalence of 22.5%in this group.
Cumulative 30-day mortality was 0.1%, 0.7%, and2.7% in patients classified as rule-out, observational zone,and rule-in (Figure 5). This pattern continued up to afollow-up of 365 days, with cumulative mortalityrates of 0.7%, 9.6%, and 8.9%, respectively
Sensitivity analysis with data removed for patientspresenting with STEMI was performed to correct for apossible bias introduced by the inclusion of patients withSTEMI because cTn and therefore the hs-cTnT 0-hour/1-hour algorithm is considered not necessary in its earlymanagement. To provide further support for the selectionof the 1-hour point for the second measurement of hscTnT, the combination of the hs-cTnT baseline level withthe 1-hour change was compared with the baseline levelalone, as well as with the combination of the baseline levelwith the 2-hour point by quantifying diagnostic accuracyby the area under the receiver operating characteristicscurve (AUC)
the hs-cTnT0-hour/1-hour algorithm assigned 78% of patients adefinite process (either rule-out or rule-in), with only 22%of patients remaining in the observational zone. Thereby,the hs-cTnT 0-hour/1-hour algorithm seemed to be evenmore complete and more effective in the early triage ofacute chest pain patients than other important emergingearly triage strategies in similar study populations
