experienced 111 Hospital
Acquired Clostridium
Difficile (C-Diff or CDI)
Infections in the 12
months from July 2015 to
June 2016. Following an
engagement with HPP
that entailed a focus on
front line staff processes
(including physicians) and
on hospital leadership
“Lean Management
Systems” they
experienced a 50%
reduction in CDI. A p-
value of 0.001
demonstrated that a
significant process
change was required to
achieve this result. See

HPP utilizes a process
where the key 1 to 3
Leading Indicators that
contribute a majority of
infections are identified.
An audit process is
employed that not only
identifies non-
compliance but in real
time corrects the
noncompliance. These
Leading Indicators are
tracked on an hourly
basis and once > 90%
compliance is achieved,
the organization begins
to realize a decrease in
infections. Lagging
Indicator(s), i.e. number
of infections, are
measured before and
after process
improvement and
changes are evaluated
for statistical
significance. A
sustainment plan offers
the organization a path
to long range
sustainment of low CDI
rates.
