Location release form

1. LOCATION RELEASE
Permission is hereby granted to Ocatve, crew and cast to enter in the property
located at Star Prodcts - 357 ChurchStreet, for the purposeof photographing
and recordingcertain scenes for a commercial. You agree that this is a
studentproduction and you willbe providingthe location with NO FEE. You
acknowledgeand you agree to the statements and conditionsstated in this
contract concerningyour Property.
Use of Star Prodctsmay use the inside and outsideof the Property located at
357 ChurchStreet on the day of Sunday 3rd of May 2015duringthehours of
9am to 4pm.
Octave willuse the Property asa location for photographing, videotaping
and/or makingsound recordings. In the eventof a delay in schedule dueto
photographically unfavorableconditions, or reasonsbeyond our control,
Producer hold the right to rescheduleat the agreed termsprovided in this
agreement.
Recordingof Footage Producer may photograph, film, videotapeand record
sound on the Property and usethe resulting materials in any way Producer
chooses.
Rights to Footage
You give uspermanent, worldwide, exclusiveright to own all rights to all
Producer recordsand photographs. You agree to have no right to inspect or
approverecordings.
Crew and Equipment
Producer may bring into the Property our crews, actors and equipment.
Producer may constructtemporary sets and, after completingphotography
and recording, will restore the Property to its original condition as of the
initial date of occupancy by Sunday 3rd of May unlessotherwise agreed to by
both parties in writing, reasonable wear and tear excepted.
Assignment
You give usthe right to assign all terms stated in this contract. You agree to
waiveany further compensation or demand of any kind forever.
Hold Harmless Producer agreesto hold you harmless from any liability and
loss which may be caused by our employeesor equipment.

2. Authority
You understand theterms described in this contract. You are over 18 yearsof
age. You have the authority to sign this contract and grant usthe rights given
under this contract.
InsuranceProducer representsthat they are covered by Public Liability and
Property DamageInsurance.
