We appreciate having you as a patient at Sunny Family Clinic. I am contacting you today in
regards to your child Mason Montgomery. We have received a physical evaluation form that
needs to be signed off and returned. For the form to be completed, we will need you to bring
Mason in. I can’t believe it has been a year already since we’ve seen each other.
The form will have to be completed before Mason can participate in the school’s basketball
program, which begins on January 3. This will help you, so that you can set time aside for this
appointment instead of being rushed at the last minute.
Please call us at 651-222-1212 to set up an appointment for the reason mentioned above. Our
office is open Monday-Friday from 8:00 a.m.- 5:00 p.m.
Please bring the following items on the day of appointment:
 Current immunization records
 Current medication
 Insurance card
 Driver’s license
It is important that you bring these documents to ensure that everything will be done smoothly
within that day. If you have any trouble locating these items, please feel free to call me, and I
