• Less reliance on in-patient admissions, delivering a
50% reduction in admissions to inpatient learning
disability services by 2020
• Developing community support and alternatives to
inpatient admission
• Prevention, early identification and early intervention
• Avoidance of crisis and better management of crisis
when it happens
• Better more fulfilled lives.
The transformation programme aims:

• People supported to move from inpatient to community settings
• On target to achieve planned bed reductions
• Transformation plan and approach supported by H&WBs and regional
OSC
• £1.2m Transformation Funding received in July to support new service
models across the Northeast & Cumbria
• Local Implementation Groups are progressing implementation of their
locality delivery plans
• Co-production Confirm and Challenge embedded within the work of the
Task and Finish Groups and Local Implementation Groups
• Community model of care developed and awaiting sign off
• Workforce development
• Financial modelling undertaken and contract negotiations underway
• Development of a Market Position Statement

• Specific work on autism including sharing the information
with Local Implementation groups to enable local
planning to reduce gaps
