Entry by Purchase: You automatically receive one sweepstakes entry form when you use your customer
Club Card and make a purchase at any participating Foodtown, Freshtown, D’Agostino’s, LaBella
Marketplace, Freshco, Brooklyn Harvest, Big Deal Food Market, Orchard Fine Foods, Uncle Giuseppe’s or
Market Fresh store during the period from January 9, 2015, through January 22, 2015. You may enter the
sweepstakes by depositing the entry form with your name and address into the ballot box at the
participating store.
Entry Without Purchase. You may enter without making a purchase by presenting your customer Club
Card at the Customer Service Desk at any participating store during the period from January 9, 2015
through January 22, 2015. NO purchase necessary. You will be given an entry form and may enter the
sweepstakes by completing and depositing the entry form into the ballot box at any participating store.
Club Card membership is free and is open to everyone who completes a customer enrollment form and has
the form processed.
Limit: One entry per household per day.
None of Sponsor, Foodtown, Inc., the respective owners of the participating stores, or their respective
employees, officers, directors, affiliates and agents assume any responsibility or liability whatsoever for
lost, misdirected, misplaced, damaged, illegible, incomplete, late or mutilated entries, or for any act or
failure to act regarding the transmittal, delay, or processing of entries or Club Card applications, or for the
operation of the Club Card program. To qualify for entry, a valid name and address associated with the
customer’s Club Card must be on file with Sponsor.
Eligibility: This sweepstakes is open to legal residents of New York, New Jersey, Connecticut and
Pennsylvania, who are 18 years or older as of January 9, 2015. Employees, officers, managers and
directors of Sponsor, Foodtown, Inc., the respective owners of the participating stores, and their affiliates,
advertising and promotional agencies and sweepstakes suppliers, including the spouses, household
members and dependents for federal tax purposes of all the above, are not eligible to participate in this
sweepstakes. This sweepstakes is being offered at participating Foodtown, Freshtown, D’Agostino’s,
LaBella Marketplace, Freshco, Brooklyn Harvest, Big Deal Food Market, Orchard Fine Foods, Uncle
Giuseppe’s or Market Fresh stores in NY, NJ and PA. This promotion is void where prohibited by law and
is subject to all federal, state, and local laws and regulations. Club Cards that are stolen, fraudulently
obtained, tampered with or otherwise altered are not eligible for entry. If multiple names are associated
with a card, the prize will be awarded to the primary cardholder. Only one winner per household.
Prizes: - One grand prize winner at each participating store will receive:
• 1 - LG 47inch LED Flat Screen TV
- One first place winner at each participating store will receive:
• 1 - 6 foot sub sandwich or the equivalent
• 1 - Buffalo Chicken Wing Platter (approx. 5 lbs.)
• 3 lbs. each – Potato Salad, Coleslaw and Macaroni Salad,
• 3 - Frito Lay chips, any variety, 9 – 16 oz. bag
• 3 - Pepsi products, any variety two-liter bottle
• 1 - Brownie Platter
Approximate retail value of all prizes - $54,375.

2. Drawing: Grand prize winners will be selected in random drawings on January 24, 2015 at approximately
3pm EST from all eligible entries. Selection of winners will be conducted by each participating store,
whose decisions are final on all matters relating to this sweepstakes. Odds of winning depend upon the
number of eligible entries from each participating store. Grand prizewinners will be notified by mail,
telephone, and/or in person. If a prize notification letter is returned as undeliverable, or if Sponsor is unable
to notify any person of his or her selection for any other reason, that person’s prize will be forfeited and
will be awarded to an alternate. All prizes are non-transferable and will be awarded in person at the
participating store. Prizes will not be delivered to winner’s home or business.
Affidavits Of Eligibility/Rights To Publish: Each prize winner selected may be required to verify
his/her identity and address, and execute and return an affidavit of eligibility and a liability/publicity
release (where permitted) within thirty (30) days of prize notification or attempted prize notification, or the
prize will be forfeited and an alternate winner will be selected. Acceptance of the prize by winner
constitutes permission for Sponsor, Foodtown, Inc. and the respective owners of the participating stores,
and their agents, to use the winner’s name, photo, voice, biographical information, and likeness for
promotional purposes in television, radio, Internet and print advertisement without further compensation or
notice, except where prohibited.
Liability: Sponsor, Foodtown, Inc., and the respective owners of the participating stores, as well as their
employees, officers, directors, affiliates, agents, assume no responsibility or liability for, and winner will be
required to agree to release, indemnify and hold said parties harmless from and against, any damages,
injuries, causes of action or losses of any kind whatsoever resulting from or in connection with the winner’s
acceptance, possession, use, or misuse of any prize mentioned herein, and against any and all liabilities
arising in whole or in part out of participation in this sweepstakes. Any and all applicable tax liability on,
or attributable to, each prize will be the sole responsibility of its winner. An IRS Form 1099 will be filed,
if required, for each prize awarded.
Winners List: For a list of winners, send a stamped, self-addressed business size envelope to Big Game
Winners List, Allegiance Retail Services, LLC, 485D Route 1 South, Suite 420, Iselin, NJ 08830. Winners
List requests must be received by March 28, 2015. Requests not fulfilling these criteria will not be
acknowledged. Allow 6 weeks for fulfillment of request.
