1. a. Report Card (form138) average of 80% and above
b. Original copy of PSA birth certificate
c. Original copy of baptismal certificate
d. 2 pcs 2” x 2” ID picture
e. Form 137 ( letter request to previous school)
f. Certificate of good Moral Character issued by the
previous school

2. ENROLLMENT AND REGISTRATION
1. Any forgery or falsification of signature in the document, or any
misrepresentation discovered by the school shall also be ground for
withdrawal of the admission( if discovered, during the first two (2) weeks
from opening of school). Or the student shall be subject to disciplinary
proceedings
3. When a student registers, it is understood that he is enrolling for the entire
year. In the event that the school authorities are forced to dismiss a student for
acts imputable to him, entire fees for the whole school year must be paid in
favor of the school.
2. No student shall be considered officially enrolled unless he/she presents all
the required documents, including the proper transfer credentials and pay all
fees required upon enrollment.
