This form enables PlaySpan to pay developers for applications they make available for a fee in
the Adobe AIR Marketplace. Please note that information you supply in Box 1 will be transmitted
to Adobe for the purpose of providing Adobe with your contact information. No other
information provided on this form will be provided to Adobe, and you agree and acknowledge
that all payment related activities are solely between you and PlaySpan. Incomplete forms will
not be processed or returned, and you agree and acknowledge that neither PlaySpan nor Adobe
will be responsible for forms that are not received.

A signed copy of one of these forms is needed in order for PlaySpan to provide tax information back to you.
4 Copy of Utility Bill
Please provide a copy of your home or business utility bill. This is used to verify your address. A utility bill for electricity is acceptable; a cell
phone bill is not acceptable.
5 Copy of Passport, Government ID, Business License or Registration/Incorporation Documents
Please provide a clear copy of a valid passport or government issued photo ID. For businesses, please provide a copy of the passport or
government ID of the primary owner AND a copy of your business license or registration documents. This is not required if the company is
listed on one of the world's major stock exchanges.

3. 6 Social Security Number or Federal Tax Identification Number
Please provide your Social Security number or Tax Identification Number. If you have both, Tax Identification Number is preferred
7 Identification of Ownership of Merchant
If you are enrolling a company, please provide a complete list of individuals that own the company and their percentage of ownership. The list
should cover 100% ownership of the company. If other companies own the company, please list the individual owners of the other companies.
This is not required if the company is listed on one of the world's major stock exchanges.

8 Terms and Conditions
You represent and warrant that (i) you have the right and authority to enter into a binding agreement on behalf of yourself
and the entity on whose behalf you are acting as a representative in connection herewith, (ii) you have the right and
authority to provide all information provided herein, (iii) all information provided is complete and accurate, and (iv) you will
immediate notify PlaySpan of any changes to the information provided herein to ensure that all information provided
