To whom it may concern,
Garret Cook has completed a Thirty-Six (36) hours precepted Acute Care Hospital clinical
rotation for CSHS Nurse Refresher program.
Garret performed her clinical rotation period in the Emergency Department. He earned high
praise for his knowledge base and his hard work from the Emergency Department Charge Nurse
as well as the Nurses she was teamed with.
He competently performed all nursing cares required for his assigned patients for Rapid
assessment, Focused reassessment, EKG and specimen collection. He performed blood glucose
checks and competently inserted a urinary catheter. He demonstrated competence in monitoring
patients with hemodynamic instability. He demonstrated competency with Wound Assessment,
measuring and treatrnent of wounds. He also demonstrated safe medication practices with the
administration of medications PO, NG and IV along with appropriate follow up documentation.
He documented care utilizing the MediTech Electronic Medical Record System. Garret also
demonstrated giving report with accuracy and thoroughness.
In addition, Garret demonstrates traits of an active learner and pursues opportunities for
leaming. He is an excellent team player and is proactive in assisting others. He has been a
pleasure to work with and will be an asset for any facility that hires him.
During the clinical rotation particrpants in the program received additional practical education as
it applies in the clinical environment for:
