2. Why is There a Need for this Project in our Community?
Adequate, safe and affordable housing is an integral factor for ensuring
improved and managed physical and mental health outcomes
People living with serious mental illnesses are dying 25 years earlier than
the rest of the population, in large part due to unmanaged physical health
conditions
More than 68% of adults with a mental disorder have at least one co-
occurring medical condition
29% of adults with a serious medical condition have a mental disorder
People with schizophrenia and bipolar disorder are three times more likely
to have three or more chronic conditions compared to people without
these disorders
Lack of housing and adequate health care leads to costly alternatives such
as incarceration, unemployment, and reliance on hospital emergency
departments for health care
How will the Wellness Community Benefit our Community?
Services provided through the Wellness Community will target health
behaviors, that increase life span and dramatically reduces overall health
care costs, for the individual and the community
Residents will have access to healthcare needs through onsite providers
and use of technology. This will reduce preventable hospitalizations
and/or readmissions
Through delivery of integrated care [primary and behavioral health],
residents will use less costly treatment services, e.g., hospital emergency

Stable housing significantly improves health outcomes and improves their
level of capability to be productive citizens
