A deficiência de hormônio tireoidiano reduz a taxa de crescimento pós-
natal e o desenvolvimento esquelético caso seu início ocorra antes do

por consequência todas as outras fases do crescimento, ou seja, criança,

tratamento precoce após o diagnóstico no período neonatal reduz

tratamento precoce do hipotireoidismo neonatal proporciona um
crescimento normal. Crescimento normal: o fator mais crítico na
avaliação do crescimento é a determinação da velocidade de
crescimento. A observação do padrão de altura da criança, o

cm/ano. Na puberdade: os esteróides sexuais (testosterona, o
estrogênio) agem em conjunto com o GH, com os hormônios da tireóide,
e com a alimentação = aceleração do crescimento = surto de crescimento
puberal. Os níveis de GH espontâneos aumentam em resposta ao
estrogênio e aos androgênios. O primeiro sinal da puberdade em
meninas (aumento das mamas) precede o primeiro sinal em meninos
(aumento testicular) por 6 meses. O surto de crescimento puberal em
meninas é 2 anos mais cedo do que em meninos (fase 3 em meninas vs
estágio 4 em meninos). A velocidade de crescimento de pico é mais lenta
em meninas (8.3cm/ano) do que em meninos (9.5cm/ano).

crescimento = aumento do comprimento dos ossos longos + taxa de
maturação esquelética. A radiografia da idade óssea (IO). Método de
avaliação de maturação esquelética, examinando maturação epifisária
da mão/pulso E <18 meses: usar raios-X heme-esqueleto devido à
imaturidade da mão/pulso. Idade óssea atrasada indica que a baixa
estatura é parcialmente reversível, porque o crescimento linear continua
até que a fusão epifisária está completa. Uma criança baixa, com idade
óssea não atrasada é de muito maior preocupação. Proporções do corpo:
a proporção do segmento superior para o segmento inferior do corpo
(tronco/pernas). Baixa estatura: proporcional (tronco e extremidades
inferiores) ou desproporcional (mais uma do que a outra). O segmento
inferior é medido pela distância entre a borda superior da sínfise púbica
e o chão em pé. O segmento superior é determinado subtraindo o
4. segmento inferior da altura de pé. Na acondroplasia, a relação
tronco/pernas será aumentada. Na baixa estatura: temos as variantes
normais; Atraso do Crescimento Constitucional Familial Baixa Estatura.
Baixa estatura patológica: diminuição do peso para a altura, doença
crônica, aumento de peso para altura; glicocorticoide- excesso de
glicocorticóides: excesso de desproporção (tronco/pernas relação ↑ ou
↓) displasias. Hipotireoidismo: congênito (sem tratamento), a
velocidade de crescimento é lenta, a idade óssea atrasada, prisão de
ventre, mentalmente atrasado, a menos que seja detectado antes da
idade de 2-3 meses. Hipotireoidismo adquirido (não tratado):
crescimento retardado, prisão de ventre, pele seca assintomática, QI
normal (se desenvolvido após 2 anos de idade).
EIXO HIPÓFISE TIREÓIDE
Independentemente dos sintomas, todas as crianças com significativa
baixa estatura devem ser rastreados para hipotireoidismo! O excesso de
glicocorticóides, iatrogênica, o tratamento farmacológico, a produção de
5. esteróides endógenos, adenoma adrenal produzindo ACTH, adenoma
pituitário, velocidade de crescimento lenta, idade óssea atrasada. O
ganho de peso aumentado e a pressão arterial elevada.

A deficiency of thyroid hormone reduces the rate of postnatal growth
and skeletal development if it’s beginning to occur before birth. In this
condition we have to take into account that both maternal and fetal
dysfunction will compromise the postnatal growth and consequently in
all other phases of growth, e.g., child, infant and juvenile. Tracking for
diagnosis of congenital hypothyroidism programs were established
worldwide. Early treatment after diagnosis in the neonatal period
markedly reduces insufficient growth and virtually eliminates mental
retardation caused by this disorder. In fact, early treatment of neonatal
hypothyroidism provides a normal growth. The normal growth: the most
critical factor in assessing growth is the determination of growth
velocity. The observation of the standard height of the child
"intersection" percentage lines in a linear growth curve that might be
the simplest method of identification of abnormal growth speed.
Repeated measurements are important with at least 3 and preferably 6
months of observation frequently. The normal growth: prenatal growth:
many factors influencing the growth and size is a function of the uterus,
maternal nutrition, insulin, and IGF/BP.
