12. Kinderen met een ernstige aangeboren afwijking moeten veelal
verschillende operaties ondergaan en hebben vaak levenslange
zorg nodig.
Nieuwe chirurgische technieken en applicaties zijn nodig om
de kwaliteit van de huidige behandelingen te verbeteren.
Eén van de veelbelovende verbeteringen is de ontwikkeling van
‘slimme biomatrices’ om beschadigd weefsel te herstellen door
lokaal de
weefselregeneratie te stimuleren.
Voor het produceren van biomatrices zijn productie technieken (clean
rooms) en nieuwe sterilisatie technieken nodig om met behulp van
de eerder ontwikkelde diermodellen het weefselregenererend
vermogen te bestuderen en in een later stadium de biomatrices in
de kliniek te evalueren.

18. Pidon Novio Tissue
Background
• The number of bladder cancer within Europe is about 90.000
patients each year. Bladder cancer that has spread to surrounding
tissue (e.g., Stage T2 tumors, Stage T3a tumors) usually requires
partial or radical removal of the bladder (cystectomy). Radical
cystectomy also involves the removal of nearby lymph nodes and
may require a urostomy (uretero-ileo-cutaneous anastomosis;
opening in the abdomen created for the discharge of urine) and an
external bag to collect urine. Complications for this current
technique include infection, urinary stones, and urine blockages.
Aim:
• Tissue engineering can be a useful tool to create alternatives for
urostomy surgery. The aim of this project is to develop and
evaluate large collagen-based tubes which might be useful to
replace intestinal tissue in an uretero-ileo-cutaneous anastomosis.
In this project, different collagen-based tubular biomatrices (Ø 3
cm) will be prepared. Then the tubular constructs will be evaluated
in a bioreactor and a pre-clinical model for urostomy.

30. • Verkleving meest voorkomende complicatie
• Zo’n negentig procent van alle mensen die een buikoperatie ondergaat,
krijgt te maken met buikverklevingen. Dat maakt het de meest
voorkomende complicatie bij buikchirurgie. Deze verklevingen kunnen
allerlei problemen veroorzaken, zoals buikpijn en darmverstoppingen. Bij
vijf procent van de patiënten treden zelfs ernstige complicaties op, zoals
beschadiging van de darm maar ook van de blaas of lever. Ook is er een
risico op onvruchtbaarheid bij vrouwen die een bekkenoperatie hebben
ondergaan. Uiteindelijk vertalen de complicaties zich in ruim veertig
miljoen aan extra ziektekosten per jaar bijvoorbeeld door extra of langere
ziekenhuisopnames en herhaalde bezoeken aan de (huis)arts.
Anti-adhesion
31. Alginaat gel
• Afgelopen twee jaar heeft EMCM een gepatenteerd proces ontwikkeld om
natrium alginaat ultra puur te formuleren op een stabiele, robuuste en goedkope
wijze. Hierdoor wordt het commercieel mogelijk medical devices te ontwikkelen
met als bestanddeel natrium alginaat. Direct gevolg hiervan is dat een anti-
verklevingsgel op basis van natrium alginaat commercieel haalbaar wordt.
• Toepassing van de gel ligt op het gebied van intra abdominale (= in de buikholte)
verklevingen na een operatie. EMCM hanteert voor deze gel de naam Rebasol.
Het werkingsmechanisme ten behoeve van anti-verkleving is al geruime tijd
bekend en is gebaseerd op het toepassen van een polyanionische biopolymeer,
zijnde natrium alginaat. Anders gezegd: door de negatieve lading van dit
biopolymeer worden verklevingen zeer effectief tegengegaan.
• Bij grote buikoperaties, inclusief laparoscopische buikoperaties, is een volume
van minimaal 200mL een absolute voorwaarde. Met Rebasol wordt dit technisch
haalbaar omdat de gel een dusdanig hoge zuiverheidsgraad kent, dat
acceptatieproblemen door het lichaam niet zullen optreden.
32. Anti-adhesion
• Verkleving meest voorkomende complicatie
• Het UMC St Radboud ontwikkelt de gel in samenwerking met regionale
partners EMCM, Bactimm BV en Doeko BV. De subsidie is uitgereikt in het
kader van PIDON, Pieken in de Delta Oost-Nederland. Dit is een
subsidieprogramma van het Ministerie van Economische Zaken ter
bevordering van de economie in Oost-Nederland. De komende maanden
vinden de laatste laboratoriumtests plaats. Vanaf januari 2011 start de
testfase bij patiënten tijdens buikoperaties.
