“Celebration of Life” Slideshow Information.
Thank you for enquiring about the “Celebration of Life” Slideshow. I would like to sincerely offer
my condolences on your lose.
This document contains information that will help you:-
1. Complete the “Celebrate the Life” Enquiry Form.
2. Help you prepare the images, music and optional video required to produce the slideshow.
By following this form MH Photography is able to give you a cost for the “Celebration of Life”
Slideshow DVD. Once you have completed the form please send by email direct to
martin@mhphotographyhampshire.co.uk or by first class mail to the address at the bottom of the
form.
If you feel that this service is of use to you, please start work on collecting the images as soon as
you can. To produce the “Celebration of Life” Slideshow DVD will require 3 clear working days. It
is important that this process is started as early as possible in gathering the images and optional
video.
The current “Celebration of Life” Slideshow packages are:-
Standard: Maximum number of images 50, music only to accompany the images with a simple
transition between the images. The slideshow will last for approximately 15-20mins.
Pick up and return of any images, is included in the price. To reflect the standard
level, the DVD box is Hard Black Plastic case and can be engraved “Family
Memories”, and will contain one DVD. Two DVD’s will be supplied, one to the client
and one to the undertaker. The second is for a backup and will be handed to the
family at the wake. £250
Enhanced: Maximum number of 75 images with a music soundtrack to accompany lasting
approximately 20mins in length. This can be enhanced with the addition of video
with music and or audio and a choice of transitions to 25-30mins. To reflect the
package level the DVD box will be a luxury folio supplied in simulated leather and
supplied in a black presentation box, with a magnetic closure and an aperture on
the front cover to hold a small image. The folio holds a single disc inside and has a
slip-in space for a photograph. Immediate family will be supplied with a copy of the
DVD. £380

Prestige: The slideshow will contain a maximum number of 100 images with music
approximately 45min in length. This can be enhanced with the addition of video
with music and or audio and a choice of transitions (this may extend the disc to
1hr). Please note that the soundtrack of the video will take preference over the
music unless specified that it is only the images that require to be viewed. To
reflect the level the DVD box will be a prestigious folio of sprung-hinged, soft
padded Cameo in an elegant book-bound style, in a black exterior finish with gold
detail, and a coloured velvet interior. Included is an inlaid gilt Cameo for inserting a
picture of the celebrant that the slideshow commemorates, a fabulous way to
enhance any DVD. The box will have a gold/silver engraved label.
Immediate family will be supplied with a copy of the DVD. £490

Client Information.
The information for the client commissioning this service is standard information. Please note that
no information given to MH Photography will be passed or sold to any other person or company.
Individuals Details
Name of the person the “Celebration of Life” slideshow is celebrating and significant dates.
Photographs
Please indicate the number of images the slideshow is to consist of. Please see information above
as to the cost.
Please consider the order that you would like the images displayed in. This is a guide for you to
consider through the various stages of their life. Parents, childhood, teenage years, work,
marriage, holidays, significant anniversaries (18, 21, 50, retirement etc.). As you compile them
please assign a number to the images i.e. 1-100 in the order that you would like them to appear.
We offer a collection service for the images for basic and standard packages, please tick the box if
you would like us to do this.
NB. You will see that a definition has been given for photos that are discoloured, creased or
scratched. This is IMPORTANT for photos that contain imperfections; this will lengthen the time
it takes to produce the DVD.
The hardest to repair are where scratches or creases are over an important part of an image. For
backgrounds i.e. sky the repair is fairly quick. To help you see what the issues are I have placed
two images at http://www.mhphotographyhampshire.co.uk/blog/ entitled “Damaged
photograph repair”.
As we load your photographs onto the PC we will check them for any problems. Those identified
as requiring major work we will advise you of any cost to repair and wait for your authorisation
before we proceed with including them in the slideshow.

Music
The choice of music is important; to play music in a public place (defined by the Performing Rights
Society (PRS) as outside the home) requires a license. The slideshow will come with a license that
complies with the copyright law that is enforced by the PRS.
The request for music tracks is to fill the sound track of the sideshow, please label the music in
order of preference (please use the reverse to add more music tracks). Please note that each piece
is between 3-4 minutes. If you choose the standard (5pieces), enhanced (7 pieces) or prestige
package (14 pieces), please list other tracks if you wish to extend this time. Please note that the
slideshow may not use all of the music.
Video
Please indicate whether or not you would like to have video footage of the individual being
celebrated, to be incorporated in the slideshow. If you already have the footage in a digital form
this will help keep the project on track.
If you have the video in another format i.e. VHS video this will be required to be converted into a
digital format and will be an additional cost. This is something that we can arrange or you can find
a company to do this for you. You may find that it is necessary to have the whole video converted
to digital media. Please note that some company’s state on their advertising, the time it will take
to convert the media.
The start time and finish time of the sequence is to identify where on the media the piece is that
you want to use and to tell us the total length of playing time.
Please note that any alterations can be added or removed up until the DVD is produced. Any
changes must be notified in writing or by email.
