2. Wound healing specialist.
 Prior to 2012, patients in the
Copenhagen area were
referred to treatment at the
CWHC by GP or other
Medical facility.
 This often prolonged
treatment by weeks or
months because of waiting
lists.

3.  After the implementation of Telemedicine in
the Copenhagen area, the district nurses are
able to refer the patients directly to the
CWHC.

4. Telemedicine
 The CWHC uses
Telemedicine to:
 Evaluate wound
healing via photo
documentation.
 Supervise the district
nurses.

5. Consultation Options
 If the patients need treatment
beyond what is possible
through Telemedicine.
 They will get an
appointment with either:
 1) The Out-Patient clinic.
 2) The Out-Function Nurse

6. Out-Function Nurse
 A Nurse specialist acting on
behalf of the CWHC in
collaboration with the district
nurses of Frederiksberg and
Copenhagen municipalities.
 Providing the benefits of the
multidisciplinary team in the
patients own home.

8. Shared Care through collaboration
between primary and secondary sector.
 Telemedicine, alone or in
combination with the Out-
function, is a valuable
addition to the public
health care system.
Especially in wound care
to the weak and elderly.
 Ensuring continuity and
save both patient and
hospital resources.

9. Conclusion
Telemedicine has contributed to:
 A dynamic treatment process.
 Shared care and responsibility.
 The patients have a sense of
being closer to the specialists.
 In the comfort of their own
home.
 Ongoing dialog about the
treatment and wound care.
 Securing a more direct and
fluent patient pathway across
sectors.
