Documentation in the medical field is a vital component to providing quality care and maintaining a consistent care plan. There must be policies in
place to protect proper documentation to maintain a high standard of care for patients. Documentation allows for a detailed history of care and displays
methods that were successful to better direct the care plan of patients. Documentation provides structure for care plans, hospital policies are care plans
for units to maintain order and provide structure for efficiency and high functionality. To maintain an optimally functioning unit there must be proper

The cardiac rehab center has two systems when charting individual interactions. Initially when a patient is admitted into the program the nurse
interviews the patient and creates a hand written care plan; the nurse manager has directed that all revisions must be on this report. The second
documentation method is electronic charting which is used in every session where the patients are supervised on a telemetry monitoring system while
exercising to make sure the patient is in safe parameters. Although the nurses write a narrative note of changes in medication, diet compliance, and
appointments; the issue lies in the fact that the information on the electronic system does not get transferred over to the physical care plan. Care plans
are unable to become revised because of missing documentation that allows the practitioner to measure successful and unsuccessful treatment. The
patients cannot receive the best care possible because of the inability to build onto previous care. In addition, other care providers cannot continue care
for patients without building a foundation for themselves of assessments because of the lack of information on the patient. With the unit having a slight
majority of elders if the center were to become audited by Medicare with the lack of documentation for each patient the program could lose

The Use Of Improper Writing Techniques For Writing
As a student, I am aware of my susceptibility to many forms of improper writing techniques that hinder my ability to produce a well–written paper.
While I was aware of most of these incorrect practices, I found myself unfamiliar with many ideas covered in the preliminary chapters of Who Says?
The Writer's Research. Prior to reading chapters 1, 3, 4, and 9, I had never acknowledged the concept of information literacy and all of its various
components. Admittedly, I had previously regarded literacy as the ability to read and write, and never thought it could be a method of interpreting
information. Furthermore, I found the many ways in which plagiarism is manifested into a writing very interesting, as I had never been aware of other
forms of plagiarism besides copying a text word for word. While I found these ideas to be of tremendous value, I found the most efficient way to
create a thesis the most helpful, as I usually struggle with crafting a singular idea when I am producing an essay. With these tools for writing, I will
now be able to act preemptively to ensure a superior essay. Chapter one introduces information literacy– a concept I found notable. Information literacy
is defined as "the ability to know when there is a need for information, to be able to identify, locate, evaluate, and effectively use that information for the

Parents Be Aware
Improper use of technology at home and school are raising questions about how this could change a child's life (Subrahmanyam). Children are allowed
to have unlimited access to their devices because parents believe these have educational benefits. This unlimited access has detrimental effects on
children. Unfortunately, these long–term effects can start to show later in adulthood. Granted, a majority of schools use technology in the classroom, but
parents are still responsible for monitoring their children's use of technology. Monitoring their technology use is important because these devices can
cause damage to the brain and the body. Children who are introduced to technology at an early age, without proper supervision, will likely suffer from
childhood obesity, lack of social skills and behavioral disorder. Researchers with The American Association of Pediatrics (AAP) are urging parents to
become aware of the long–term effects of screen time. These effects are slowly developing over time. One impact is childhood obesity (Chan). A
child will start to develop symptoms of childhood obesity because of poor eating habits, lack of physical activity and targeted marketing advertising
from the media. Experimental laboratory studies have claimed that TV viewing is associated with unhealthy eating habits, which is a contribution to
obesity. These unhealthy snacks can later cause higher sugar levels and be stored as body fat. Chan from Harvard University is recommending that
children should spend 2–3 hours a day performing a physical activity because children who spent more time in TV viewing had an increase in their
body mass index (BMI) rate(Chan). The food industry is also using media to target children as their consumers. Targeted food marketing can cause
children to choose from unhealthy food options while parents may be unaware of the influence on their children's unhealthy food choices (Chan).
These unhealthy snacks and food choices will only continue as a pattern if parents allow this practice as a habit. Spending too much screen time is an
unhealthy habit, no child should suffer from obesity because of technology.

low income families. These programs are designed to provide a safety net to protect people from poverty. There are thirteen different welfare programs
that will help assist people by providing necessities like food, and housing. But today many have seen welfare as not doing anything to benefit the
