Change of this kind will require the harnessing of the sector’s long tradition of voluntary and community actions”

  6. 6. What is the user journey? • Self-assessment or supported assessment • Informed of amount to met outcomes (An outcome is something that a person ants to achieve or aim for that will improve their social care needs). • Self or supported care plan of support • Once agreed, given budget
