  18. 18. Significant Events February 2011 Child subject to Child Protection Plan Father attempted suicide Mother from family previously bereaved by suicide Mothers increased risk raised to case conference team. Circumstances re-assessed – case conference updated.
  19. 19. Significant Events April 2011 Person continually presents on bridges threatening to jump Treated as ‘time waster’ when taken to A&E and no treatment given Obviously being let down by system GMP and Samaritans work together to give an alternative option at crisis point
