  5. 5. At Pak Suzuki, we focus accident / incident investigation based of thestandard Heinrich’s Law. According to this law out of 330 times in anidentical situation, there were 300 near miss-accidents or opportunities toforecast an accident 29 times there was a minor accident, and 1 out of the330 was a serious accident.

All accidents have multiplecauses i.e.• Primary Causes•Secondary causes•Contributing causesIf we can reduce the causesand eliminate even one of Primarythem, we can prevent same Secondarynature of accident from Cause Causehappening it again. ContributingThe key is to investigate thetrue cause of the accident Causesthrough why-why method.

WHY METHOD OF ACCIDENT INVESTIGATION Why, Why AnalysisThis method starts out very Cause of Accident Accident Injurysimply (I) name of the injured Injuryperson (2) nature of injury and(3) element that caused theinjury. Then we start with the Why ?Cause of Injury and ask the Why ?question why? Then repeat by Why ?asking why to the answer of thatquestion until there is a series of Why ?questions and answers. Fromthat list, we select the Primary Why ?Cause that is most likely.

In Pak Suzuki Safety system is based on the thought that we can takecomplex systems and break them down into smaller inter-related systemswith relationships and effectively deal with problems. By identifying potential hazards Incorporate safety into design (Built-in Safety) Evaluate the designs before time By monitoring all safety aspects.
