Abstract
Thai medical students during their clinical years spend an excessive amount of time
standing in hospital wards, out-patient departments, and operating theatres. From the authors’
observation, a large proportion of medical students report low back pain (LBP); a debilitating
condition shown to have short-term and long-term physical and psychological effects. Excessive
standing itself causes many unpleasant conditions, one of them being LBP. The aim of this cross-
sectional study is to examine the relationship between excessive standing and LBP in 5th
year
medical students in order to develop a countermeasure against the effects of LBP and improve the
overall health of medical students. Data were collected from 154 5th
year medical students at
Srinakharinwirot University in October 2015 using an online questionnaire. The questions asked
included time spent standing each day, sex, BMI, job satisfaction, standing posture, type of shoe,
history of chronic headache, history of back injury, exercise, heavy lifting, chronic use of analgesic
drugs, and the Oswestry Disability Index. Categorical data was analysed using the Chi-square test,
and continuous data was analysed using the Mann-Whitney U test. Confounding factors were
analysed using multiple logistic regression with a p-value < 0.05 out of 99 medical students (32.3%)
developed LBP after standing for at least 2 hours. On the contrary, out of 55 medical students
(5.5%) reported having the same symptom after standing less than 2 hours. There was a significant
relationship between standing for 2 hours or more per day and LBP (Adjusted OR 7.244 [95% CI
2.203-25.938]; p-value = 0.002). Additionally, job dissatisfaction was found to have a significant
relationship with LBP (Adjusted OR 7.722 [95% CI 2.314-25.768]; p-value = 0.001). The results of this
study suggests that standing for 2 hours or more per day as along with job dissatisfaction resulted in
an increase in the prevalence of LBP in 5th
year medical students at Srinakharinwirot University.
