ORIGIN AND BRIEF HISTORY OF INSTITUTION
The VETERANS MEMORIAL MEDICAL CENTER was constructed as a gift of the
American people to the Philippines as token of gratitude to the Filipino soldiers who
stood by them during World War II. The 80th Congress of the United States of
America passed Public Law 865 in July 8, 1948 which provided $9.4 million for the
construction of a hospital facility with the land to be donated by the Filipino people.
The law was implemented in the country by an agreement between the U.S. and the
Philippines signed by then Pres. Elpidio Quirino and the U.S. Ambassador to the
Philippines Myron Cowen. Hospitalization is extended to veterans of World War II
and their dependents under the RP program. With the passing of RA# 6948, AFP
retirees and their dependents are extended benefits of the Public Law.
All that a patient needs to present in seeking admission is a certificate of his military
service from the Philippine Veterans Board and this plus his marriage certificate and
birth certificate of minor child if the dependent is in need of hospital care.
Emergency cases, however, may be brought to the hospital directly after which
pertinent papers may be secured later on.
The Veterans Memorial Medical Center was established pursuant to Public Law 865,
80th U.S. Congress. Various amendments have been introduced and the latest is
Republic Act 6948 which has liberalized the definition of veterans which now
included the Armed Forces of the Philippines (AFP) retirees. Likewise, the
hospitalization benefit was extended to the veteran dependents. It is one of three
units under the Philippine Veterans Affairs Office (PVAO). PVAO is a bureau under
the Department of National Defense. The other two units under PVAO are the PVAO
Proper, and Military Shrines Service.
The 55 hectares (140 acres) land was originally intended to be location of the new
Malacanang Palace on the original master plan of Quezon City in 1939. It is now
being eyed to be privatized as part of the Quezon City Central Business District
Redevelopment Plan or Triangle Park.

 Coordination of program plan with Veterans Memorial Medical Center
hospital Director and Administrator.
 Set program date of wheelchair donation to VMMC.
 Donation day with VMMC and its beneficiaries.
PART C: INTERNAL AUDITING
The wheelchairs will be donated to Veterans Memorial Medical Center and will be
audited by the Administrative Office under the supervision of the Department of
Family Medicine Outpatient Department. The donated wheelchairs will be utilized by
patients covered by the Outpatient Department.

III. Aim of Proposal
To provide assistive device such as wheel chairs to geriatric patients with disability
and those that are recovering from medical procedure.
Improve the quality of life and provide ease to geriatric patients who need assistance
in going to various departments of the institution.

IV. Objectives of the Proposal
Geriatric patients at one point or another will need some help getting around.
Whether they have broken a leg, become paralyzed, or are recovering from a
medical procedure, the quality of life will improve when one finds a method to move
around at will. One common method is to use a wheelchair, which offers many
advantages
Paralysis
A geriatric patient who is paralyzed and unable to use his legs for walking can sit in a
wheelchair and use it to move around. There are manual wheelchairs for people who
can use their hands and arms to push the wheels. Electric models provide the power
at the touch of a button. In cases where the arms are also paralyzed, a person can
use other methods to operate the wheelchair, such as a mouth-operated blow tube,
chin controls or head-movement controls.
Fractures
When a person breaks her leg, knee or ankle, the doctor will advise against putting
weight and stress on it while the bone mends. Sometimes she can get around on
crutches, but using them for long periods may be too painful or exhausting. When
both legs are broken, crutches are not an option. A wheelchair gives her the
advantage of being able to move around for as long and as far as she likes.
Hospitalization
A patient recovering from surgery or illness in the hospital may be too weak to walk
very far, and will benefit from using a wheelchair to move around. Until anesthesia
wears off, a person may get dizzy and fall down if he attempts to walk, so he will use
a wheelchair to go places. Another advantage of a wheelchair is to provide safety
and comfort to a maternity patient arriving at the hospital during labor.
Balance
A person who can walk but has difficulty keeping balance might have to reduce the
distance and amount of time spent walking, or risk injury from falling down. One
advantage of using a wheelchair is that a person with balance issues can continue

doing regular activities. He can go shopping or clear snow from the driveway without
worrying about getting hurt.

1. The Philippine Revolutionary Forces that took up arms against the Spaniards and the
Americans during the past Philippine revolutions and wars,
2. members of the First Philippine National Guards, either before or after its integration
into the Army of the United States of America,
3. Filipino citizens who are enlisted and or commissioned and served during World War
I under the Allied Armed Forces,
4. Philippine Scouts who were in the service prior to October 6, 1945, with wartime
service,
5. USAFFE with wartime service in the Philippines,
6. Recognized World War II guerrilla units in the Philippines and
7. AFP units that participated in the Korean War.

II. EXECUTIVE SUMMARY
SIGNIFICANCE OF THE PROJECT
Veterans Memorial Medical Center was chosen to have the project implemented as an
appreciation to our veteran soldiers regardless of their service and the era in which they
have served, these veteran soldiers and their beneficiaries have paid the price time and time
again, defended our country through both the best and the worst times and have performed
their duties tirelessly all those years.
This is also part of the Corporate Social Responsibility’s subject’s curriculum where the class
has decided to donate 7 PCS BRAND NEW WHEELCHAIRS under the “AKBAY KAWAL
PROGRAM” which aims also to provide the Department of Family Medicine Out-Patient
Section with the much needed support.

ATTENDEES OF THE PROJECT
The project was attended by 23 students of CSR class First Semester SY 2014-2015 scheduled
every Saturday 2:30pm – 5:30pm under the supervision of Dr. Danilo Reyes. The class was
also joined by the representatives of the Out Patient Section of VMMC from the Department
of Family Medicine and Supply Division.
HIGHLIGHTS OF THE PROJECT
The highlight of the project was during the turn-over of the 7pcs wheelchair to Veterans
Memorial Medical Center which aims to provide assistive medical equipment which was also
witnessed by the geriatric patients of the Out-Patients Section of the hospital.
III. ACCOMPLISHMENT OF OBJECTIVES

V. CHALLENGES AND ACTION PLANS
 Initially, the class has a problem with their corresponding work schedule with regard
to the date on when the project will be implemented. As discussed, the class has
decided to do the implementation on the last week of September to ensure that
everyone will be available to attend the said activity.
 The collection for the budget that will be allocated to the project was not collected
all at the same time. Class representative Dinah Ladao opted to have the remaining
collection deposited to her bank account during weekdays in consideration for those
that were not able to bring the amount. Once collection was completed, it was
handover to Victoria Caballero who was assigned to take charge with regard to the
purchasing of the equipment.
 The purchasing of the wheelchairs was delayed for a week in consideration for the
completion of the amount to be collected from the students for the CSR project.
With this, the amount of the wheelchairs was affected following the congestion
problem at the port area where the wheelchairs are bought by the supplier. The

class was asked to add a minimum amount as an adjustment for the said unforeseen
circumstance which was fully paid by the whole class on the day of the activity.
VI. OTHER INSIGHTS/RECOMMENDATIONS
Veterans Memorial Medical Center has been servicing veteran geriatric patients and its
beneficiaries since 1950’s. The institution is currently under the supervision of the Philippine
Veterans Affairs Office located at the Camp General Emilio Aguinaldo, Quezon City which is
currently not a main priority of the government. In fact, budget allocations through the
years have decreased which is affecting some services provided by the hospital.
Corporate Social Responsibility is a way for people to reach out to others, provide additional
help to the government in terms of social assistance to those beneficiaries who are in need
of help on a regular basis. The “Akbay Kawal Program” should be continued by other
corporations and NGO’s to ensure that our Philippine Veterans are provided the love and
care they deserve for sacrificing so much for our country. It should be a priority not just by
the government but also by the public to ensure that they are provided with their needs.

CORPORATE SOCIAL RESPONSIBILITY 2014 4 Document Prepared & Created by: Victoria Jesusa Caballero ORIGIN AND BRIEF HISTORY OF INSTITUTION The VETERANS MEMORIAL MEDICAL CENTER was constructed as a gift of the American people to the Philippines as token of gratitude to the Filipino soldiers who stood by them during World War II. The 80th Congress of the United States of America passed Public Law 865 in July 8, 1948 which provided $9.4 million for the construction of a hospital facility with the land to be donated by the Filipino people. The law was implemented in the country by an agreement between the U.S. and the Philippines signed by then Pres. Elpidio Quirino and the U.S. Ambassador to the Philippines Myron Cowen. Hospitalization is extended to veterans of World War II and their dependents under the RP program. With the passing of RA# 6948, AFP retirees and their dependents are extended benefits of the Public Law. All that a patient needs to present in seeking admission is a certificate of his military service from the Philippine Veterans Board and this plus his marriage certificate and birth certificate of minor child if the dependent is in need of hospital care. Emergency cases, however, may be brought to the hospital directly after which pertinent papers may be secured later on. The Veterans Memorial Medical Center was established pursuant to Public Law 865, 80th U.S. Congress. Various amendments have been introduced and the latest is Republic Act 6948 which has liberalized the definition of veterans which now included the Armed Forces of the Philippines (AFP) retirees. Likewise, the hospitalization benefit was extended to the veteran dependents. It is one of three units under the Philippine Veterans Affairs Office (PVAO). PVAO is a bureau under the Department of National Defense. The other two units under PVAO are the PVAO Proper, and Military Shrines Service. The 55 hectares (140 acres) land was originally intended to be location of the new Malacanang Palace on the original master plan of Quezon City in 1939. It is now being eyed to be privatized as part of the Quezon City Central Business District Redevelopment Plan or Triangle Park.

PART C: INTERNAL AUDITING The wheelchairs will be donated to Veterans Memorial Medical Center and will be audited by the Administrative Office under the supervision of the Department of Family Medicine Outpatient Department. The donated wheelchairs will be utilized by patients covered by the Outpatient Department. PART D: PROJECT DETAILS I. Improve the quality of life and provide ease to geriatric patients who need assistance in going to various departments of the institution.

Objectives of the Proposal Geriatric patients at one point or another will need some help getting around. Whether they have broken a leg, become paralyzed, or are recovering from a medical procedure, the quality of life will improve when one finds a method to move around at will. One common method is to use a wheelchair, which offers many advantages Paralysis A geriatric patient who is paralyzed and unable to use his legs for walking can sit in a wheelchair and use it to move around. There are manual wheelchairs for people who can use their hands and arms to push the wheels. Electric models provide the power at the touch of a button. In cases where the arms are also paralyzed, a person can use other methods to operate the wheelchair, such as a mouth-operated blow tube, chin controls or head-movement controls. Fractures When a person breaks her leg, knee or ankle, the doctor will advise against putting weight and stress on it while the bone mends. Sometimes she can get around on crutches, but using them for long periods may be too painful or exhausting. When both legs are broken, crutches are not an option. A wheelchair gives her the advantage of being able to move around for as long and as far as she likes. Hospitalization A patient recovering from surgery or illness in the hospital may be too weak to walk very far, and will benefit from using a wheelchair to move around. Until anesthesia wears off, a person may get dizzy and fall down if he attempts to walk, so he will use a wheelchair to go places. Another advantage of a wheelchair is to provide safety and comfort to a maternity patient arriving at the hospital during labor. Balance A person who can walk but has difficulty keeping balance might have to reduce the distance and amount of time spent walking, or risk injury from falling down. One advantage of using a wheelchair is that a person with balance issues can continue

Philippine Scouts who were in the service prior to October 6, 1945, with wartime service, 5. USAFFE with wartime service in the Philippines, 6. Recognized World War II guerrilla units in the Philippines and 7. AFP units that participated in the Korean War. EXECUTIVE SUMMARY SIGNIFICANCE OF THE PROJECT Veterans Memorial Medical Center was chosen to have the project implemented as an appreciation to our veteran soldiers regardless of their service and the era in which they have served, these veteran soldiers and their beneficiaries have paid the price time and time again, defended our country through both the best and the worst times and have performed their duties tirelessly all those years. This is also part of the Corporate Social Responsibility’s subject’s curriculum where the class has decided to donate 7 PCS BRAND NEW WHEELCHAIRS under the “AKBAY KAWAL PROGRAM” which aims also to provide the Department of Family Medicine Out-Patient Section with the much needed support. The class was also joined by the representatives of the Out Patient Section of VMMC from the Department of Family Medicine and Supply Division. HIGHLIGHTS OF THE PROJECT The highlight of the project was during the turn-over of the 7pcs wheelchair to Veterans Memorial Medical Center which aims to provide assistive medical equipment which was also witnessed by the geriatric patients of the Out-Patients Section of the hospital. III. ACCOMPLISHMENT OF OBJECTIVES OBJECTIVES STRATEGIES ACTIVITIES RESULTS To provide assistive device such as wheel chairs to geriatric patients with disability and those that are recovering from medical procedure. Purchasing of the wheelchairs ensuring that the best quality equipment was found and yet budgetary prohibitions was maximized for the benefit of both the beneficiaries and benefactors of the project. Hand-over of the 7pcs wheelchairs to the Department of Family Medicine Out-Patient Section and Supply Division Objective was attained and the hand-over of the wheelchairs was a success providing guarantee that the equipments donated are accounted for as well as assured that everything is serviceable for the benefit of the geriatric patients Improve the quality of life and provide ease to geriatric patients who need assistance in going to various departments of the institution. Purchased wheelchairs are provided directly to the Out-Patient Section of VMMC as they have the most number of patients per month as compared to the main section of the hospital Hand-over of the 7pcs wheelchairs to the Department of Family Medicine Out-Patient Section and Supply Division With the quality of the equipment donated to VMMC OPS, veteran patients are assured that the equipments donated will improve the quality of life and ease to patients who need assistance in going to various

CHALLENGES AND ACTION PLANS  Initially, the class has a problem with their corresponding work schedule with regard to the date on when the project will be implemented. As discussed, the class has decided to do the implementation on the last week of September to ensure that everyone will be available to attend the said activity.  The collection for the budget that will be allocated to the project was not collected all at the same time. Class representative Dinah Ladao opted to have the remaining collection deposited to her bank account during weekdays in consideration for those that were not able to bring the amount. Once collection was completed, it was handover to Victoria Caballero who was assigned to take charge with regard to the purchasing of the equipment.  The purchasing of the wheelchairs was delayed for a week in consideration for the completion of the amount to be collected from the students for the CSR project. With this, the amount of the wheelchairs was affected following the congestion problem at the port area where the wheelchairs are bought by the supplier. The

VI. OTHER INSIGHTS/RECOMMENDATIONS Veterans Memorial Medical Center has been servicing veteran geriatric patients and its beneficiaries since 1950’s. The institution is currently under the supervision of the Philippine Veterans Affairs Office located at the Camp General Emilio Aguinaldo, Quezon City which is currently not a main priority of the government. In fact, budget allocations through the years have decreased which is affecting some services provided by the hospital. Corporate Social Responsibility is a way for people to reach out to others, provide additional help to the government in terms of social assistance to those beneficiaries who are in need of help on a regular basis. The “Akbay Kawal Program” should be continued by other corporations and NGO’s to ensure that our Philippine Veterans are provided the love and care they deserve for sacrificing so much for our country. It should be a priority not just by the government but also by the public to ensure that they are provided with their needs.
