Classroom Assessment
Formative & Summative Assessment
6.
Two Types ofAssessment
DepEd Order No. 015, s. 2026 | Revised Classroom Assessment Guidelines
FORMATIVE ASSESSMENT
Embedded in DAILY instruction
NOT used for grade computation
Purpose: Improve learning
Provides timely feedback
ESRU Model: Elicit → Student
Response → Recognize → Use
Used before, during & after lessons
SUMMATIVE ASSESSMENT
USED for grade computation
Three components: WWs, PTs, EXs
Written/Oral Works (WWs)
Product/Performance Tasks (PTs)
Examinations: STs + Term Exam (TE)
Quality over quantity — avoid
overload
7.
Recommended Number ofSummative Assessments (Table 3)
DepEd Order No. 015, s. 2026 | Revised Classroom Assessment Guidelines
Summative
Assessment
Grades 1–3 Grades 4–12
Written/Oral Works
(WWs)
Teacher has discretion
(sufficient evidence,
avoid overload)
3–5 WWs per learning
area, per term
Product/ Performance
Tasks (PTs)
Teacher has discretion

KS3 (Grades 7–10) Required; includes higher-order
thinking skills 50 items
KS4 (Grades 11–12) Required; competency-based and
discipline-specific 60 items
*Refers to number of test items; total points may vary based on item type & guided by the TOS.

Qualitative Descriptors ofNumerical Grades (Table 11)
DepEd Order No. 015, s. 2026 | Revised Classroom Assessment Guidelines
Numerical
Grade Descriptor General Description Instructional Response
90–100 Advancing
(Namumukod-tangi)
Consistently demonstrates skills and
understanding; exceeds standards
Enrichment, leadership,
peer mentoring
80–89 Benchmarking
(Napamamalas)
Demonstrates expected grade-level
skills competently and independently
Deeper application,
transfer of learning
75–79 Connecting
(Natutungo)
Demonstrates sufficient understanding;
occasional guidance needed
Guided practice to
strengthen consistency
65–74 Developing
(Napauunlad)
Partial understanding; inconsistent
application
Targeted remediation,
scaffolded instruction
0–64 Emerging
(Nagsisimula)
Does not yet demonstrate foundational
skills; requires intensive support
Structured and sustained
intervention programs
✅ Final Grade of 75 or higher = PASSED ❌ Below 75 = requires remedial intervention
14.
How to Computethe Grade
(Annex D)
1 Get Total Raw Scores (RS) Sum all raw scores from WWs, PTs, and EXs.
2
Convert to Percentage
Score (PS)
PS = (RS ÷ Highest Possible Score) × 100
3
Compute Weighted Score
(WS)
WS = PS × Assigned Weight (e.g., 20%, 50%, 30%)
4
Add All WSs → Initial
Grade (IG)
Sum of all Weighted Scores = Initial Grade for the term.
5
Apply Transmutation
Table (SY 2026–27)
IG of 70.00 → Transmuted Grade of 75 (adjusted table).
Beginning SY 2027–28, no transmutation — Zero-Based.
The Final Grade (FG) = Average of all Term Grades, rounded to nearest whole number.

Promotion, Retention &Intervention Rules
DepEd Order No. 015, s. 2026 | Revised Classroom Assessment Guidelines
✅ PROMOTED
Learner passes ALL learning areas in the
previous grade level. FG of 75 or higher in
each subject.
📚 SUMMER REMEDIAL CLASS (SRC)
For learners who fail in at most TWO (2)
learning areas. SRC Mark averaged with FG =
Recomputed FG. If RFG ≥ 75, learner is
promoted.
📌 CONDITIONAL PROMOTION
Fail in 1–2 areas after SRC → Conditionally
promoted. Completes deficiency as a back
subject through School-Initiated Intervention
(SII).
❌ RETAINED
Learner fails MORE THAN TWO (2) learning
areas. Must repeat the same grade level.
⚠️Excessive absences (>20% of class days) = Failing grade & no credit, unless School Head grants exemption.
📋 SHS: Failure in a prerequisite prevents enrollment in higher-level subjects until remediation is complete.

Key Takeaways —What This Means for You
DepEd Order No. 015, s. 2026 | Revised Classroom Assessment Guidelines
👩
👧
PARENTS
Grades are based on fair, transparent evidence — not just tests.
KS1 (K–Grade 3) now uses descriptive grades (letters A–E), not
numbers.
Grade reports (SF9) will include narrative feedback for KS1.
Your child needs ≥75 to pass; Summer Remedial Class
available if they fail 1–2 subjects.
Academic Excellence Award: GA ≥90, no FG below 80.

🎒 STUDENTS
Formative tasks (class activities,
reflections) do NOT affect your grade.
Performance Tasks (PTs) carry the
most weight — do your best!
AI use in exams is PROHIBITED.
Limited/guided use in other tasks
must be disclosed.
Fail ≤2 subjects → Summer Remedial
Class. Fail >2 → repeat grade level.
Awards are listed alphabetically — no
ranking or 1st/2nd/3rd honors.
🏫
TEACHERS
Formative assessment: embedded daily
using ESRU model — NOT graded.
Keep assessments manageable: 3–5
WWs, 2–3 PTs, 2 STs + 1 TE per term.
Maintain complete, accurate class
records — validated by school heads.
Apply adjusted transmutation table (SY
2026–27); zero-based grading from SY
2027–28.
AI for design only (limited/guided);
never for determining Final Grades.

Self-Reality Check on
Classroom Implementation
.
The Department of Education provides standards and policies to
ensure quality, equitable, and learner-centered instruction across all
schools. This instrument is not intended to identify right or wrong
practices. Instead, it provides an opportunity for teachers to reflect
honestly on how classroom instruction is actually carried out in
everyday practice, recognizing the realities described in Street-Level
Bureaucracy.
