MADDOX ROD:
A Maddox rod is a clinical tool used in eye examination.
To assess and diagnose abnormalities in binocular vision.
It consists of a series of parallel cylindrical lenses (rods) arranged in a row,
usually made of transparent plastic or glass.
DOUBLE MADDOX ROD:
Test is a specialized variation of standard Maddox rod test used to assess
and quantify the presence of cyclo-deviation; Which is a type of torsional
misalignment of eyes.
Cyclo-deviation occurs when the eyes rotate around their visual axes
leading to a tilting or twisting of images seen by each eye.
HOW IT WORKS:
In double Maddox rod test;
Two Maddox rods are used, one in front of each eye.
Typically, the rods are oriented so that the cylindrical lenses are at the
same angle, often at 90 degrees (vertical)
Making the light appear as a horizontal line in both eyes.
PROCEDURE:
1. The patient is asked to look at a point light source while both Maddox
rods are in place.
2. Each eye will see the light as a line. In the presence of cyclo-deviation
the lines seen by each eye will not be parallel to each other.
3. The patient or examiner can rotate the Maddox rods until the lines
appear parallel to the patient.
4. The amount of rotation needed to achieve this provides a measure of
the degree of cyclo-deviation.
INTERPRETATION:
Incyclotorsion:
If the lines seen by the eyes are tilted towards the nose, this indicates
incyclotorsion.
Excyclotorsion:
If the lines are tilted towards the temples, it indicates excyclotorsion.
The amount of rotation required to make lines parallel is measured in
degrees and reflect the extent of torsional misalignment.
CLINICAL RELEVANCE
The double Maddox rod test is particularly useful in diagnosing and
managing conditions like
Fourth nerve palsy (superior oblique muscle palsy )
Other disorders that cause torsional diplopia (double vision)
 cyclophoria
