Online publishing permission slip to parents

  1. 1. September 18, 2009Dear Parent or Guardian: Publishing is the final stage of the writing process, and it is important because it givesauthors a purpose for writing: sharing their ideas with an audience. So, this semester, Ms.Chapman’s and Mrs. Voorhees’s 7th/8th Grade Language Arts classes as well as other Savagestudents, will collectively publish a book titled Off the Blacktop and Onto the Page. Yourpermission is required in order to have your child’s work included in this book. Please reviewyour child’s work (listed below and attached) before signing this permission slip. Make sure thatyou are comfortable with your child publishing this work. The book, once published, will beavailable for anyone in the world to purchase, so private information appearing in your child’swork should be altered or deleted to maintain privacy. Discuss these and other changes with yourchild. The copyright of the individual works remains in the hands of each student. You aremerely giving permission to have your child’s work published in this edition of Off the Blacktopand Onto the Page. Students can publish their own work in the future however they see fit. Proceeds from the sale of the book are donated to The Daniel J. Savage Book Club. I willpurchase a copy for our classroom, so you are not required to buy anything. However, I inviteyou to order a copy of Off the Blacktop and Onto the Page for your child from www.lulu.com . Sincerely, Ms. Chapman and Mrs. VoorheesI give permission to allow my child, _________________________________________, tosubmit his or her work titled “______________________________________________,”(attached) for publishing as part of book containing a collection of student works. My child will: (check one) □ Use his or her real first and last name. □ Use his or her first name only. □ Use this pseudonym: _____________________________ _________.Parent Signature: _____________________________ (date) __________Student Signature: _____________________________ (date) __________Print Student Name: ___________________________________________
