This plan provides extensive dental care with eligible restorative services covered up to $1,500 when visiting a Cigna DPPO Advantage provider. • Covers eligible preventive, basic and major restorative services and orthodontia after waiting periods are met • Waiting period is waived for covered preventive services • Annual deductible is waived for preventive and diagnostic services, which helps to lower customer’s expenses • 100% coverage for up to two cleanings a year and routine x-rays** • Waiting periods for Classes II and III are waived for those with 12 months of qualified continuous prior insurance coverage • Waiting period is not waived for orthodontia • When using Cigna DPPO Advantage providers, customers gain access to network discounts on covered services even if they are still in their waiting period and after annual maximums have been met • A separate lifetime deductible and benefit maximum applies to orthodontia • This plan is best suited for customers looking for comprehensive coverage with a higher annual benefit maximum Positioning Features Agent/Broker Use Only *Invisalign is covered only when orthodontia is a covered class on DPPO. **For eligible services when you see an in-network provider. With this plan, Cigna pays 100%* of preventive care dental visits when visiting a Cigna DPPO Advantage provider. • Has an average monthly premium of approximately $17** • This plan will appeal to those seeking basic dental coverage at a lower cost or those that have good oral health and are in “maintenance mode” • This is an entry plan for those that do not currently have dental insurance and would be subject to waiting periods on the Cigna Dental 1000 and 1500 Plans • $0 deductible • 100% coverage for up to two cleanings per consecutive 6 months and routine x-rays*** • Network discounts for Classes II, III & IV with Cigna DPPO Advantage dentists (except in VA and MD) • Roughly $15 - $20* less expensive (per Month) than the 1500 Plan per person • Roughly $10 - $12* less expensive (per Month) than the 1000 Plan per person Positioning Features Agent/Broker Use Only *Some preventive expenses may not be covered, like athletic mouth guards. Use and distribution limited solely to authorized personnel. © 2016 Cigna Cigna's Individual Dental plans require customers to satisfy waiting periods before the plan will cover basic and major restorative dental services (Classes II and III) • Services can be provided by the dentist but customers will be responsible for payments Waiting periods are waived only if: The coverage was an insurance product – including preventive plans – but was not a discount plan (Cigna’s Preventive Plan is Creditable Coverage) Twelve months of qualifying continuous coverage was in place within the 63 days prior to the effective date of this coverage - AND - - AND - The customer has a full twelve months of qualifying continuous dental insurance coverage • Waiting periods are not waived for orthodontic coverage (Class IV) • Except in MO and RI • Waiting periods are waived for preventive services • In completing their application for coverage, customers are asked to indicate whether or not they have had prior coverage – customer will need to identify with whom they had coverage, and for how long Things to note: Agent/Broker Use Only

Dependents are covered through the end of the month during which they reach their state’s maximum dependent age limit. • CT = to age 27 • FL & IL = to age 30 • All other states = to age 26 For example: • Family purchases a policy on 3/12/16 with an effective date of 4/1/16 • Customer has a dependent turning 26 on 10/10/16 • Dependent will have coverage through 10/31/16, and will be canceled at the end of October • Cigna will send a letter to the customer 60 days before the dependent ages out to offer the opportunity for the dependent to obtain a policy of their own through broker An alternative example: • Family purchases a policy on 3/12/16 with an effective date of 4/1/16 • Customer has a dependent turning 26 on 3/30/16 • This customer will NOT be treated as a dependent because they are 26 as of the effective date • They will need to purchase their own policy • Note: Make the customer aware if the dependent child age is close to the age out requirement during enrollment Agent/Broker Use Only Enrollment

Can a customer enroll in a 1500 plan and then switch to the 1000 or preventive plan a few months later? A. No, this is not allowed as it is a ‘downgrade’. Customers should purchase the level of coverage they anticipate they will need and not wait to downgrade their plan when less coverage is needed. Q.
