  2. 2. 70 yr old male presented with h/o swelling in rt lower lid -2months<br /> Painless swelling,slowly increasing in size.<br />No h/o trauma,no h/o discharge from the mass.<br />H/o similar swelling at the same site noticed 1yr back which was treated by aspiration 3 months back<br />

H/o DM-4yrs<br />General examination and systemic examination wnl<br />H/o cataract surgery in BE.<br />BCVA = 6/12 in BE.<br />10 x 12 mm localized tense cystic swelling in lateral 1/3rd of RLL,exendingupto lateral canthus<br />Overlying skin shows few dilated vessels with yellowish deposits in mass.<br />
  4. 4. Cyst wall was continuous with LL margin<br />Soft, non tender, nonpulsatile, non compressible, nonreducible ,freely mobile mass.<br />No conjunctival swelling noted.<br />Otherwise antr segment and postr segment,wnl. <br />

  6. 6. Clinically d/d considered were,<br />? Implantation cyst<br />? Large moll’s cyst<br />? Parasitic cyst.<br />BRE and URE-wnl.<br />“Excision of cyst with primary reconstruction of RUL”done under LA.<br />Aspirated fluid from mass,was turbid with yellowish white coagulam. <br />

Dermoid cysts<br />Are choristomas,tissue anomalies c/d by types of cells not normally found at the involved site.<br />Though,derived from all three germinal layers,ocular dermoids consists mainly ectoderm with mesodermal elements.<br />Account for 3-9% of all orbital masses.<br />About 50% of trs involving head are found in/adjascent to orbit.<br />
  14. 14. Can be found in all age groups,but most commonly in adolescent age groups.<br />Most frequent site - supero temporal quadrant of orbit [in relation to frontozygomatic suture].<br />In eye,other sites<br /> are limbus,<br />eyebrows,<br />conjunctiva and<br /> rarely eye lids. <br />
  15. 15. <ul><li>In lids,upper lids are commonly involved.only 2 cases were reported to arise from lower lids.</li></li></ul><li>Reported unusual presentations of lldermoids,previously were<br />CASE REPORT-1<br />Orbital floor dermoid in an 18 yr old female,presented with painless swelling in LL,a/w mild proptosis of 3 months duration.<br />pt underwent total excision of the lesion,through an incision along the inferior orbital margin. Histopathology confirmed the diagnosis of dermoid cyst. <br />[yeola et al,IJO 2009;57:51-2]<br />

CASE REPORT-2<br />10 yr-boy with a painless swelling in nasal side of LLL of 5 yrs duration,<br />Cthead- showed intracranial extension <br />Only orbital part of mass was removed by transfrontalethmoidectomy with medial orbitotomy.<br />Histopahologyrevealed dermoid cyst.<br />[Srivasthava u et al,ijo 2004;52:244]<br />

  19. 19. CT and MRI imaging done in suspected cases of intracranial and intraorbital extension.<br />show thicker cystwall d/t dermal adnexa.lining can calcify and may enhance with contrast material.lipid material inside show attenuation and signal intensity that of fat.<br />
  20. 20. TREATMENT<br />Surgical enbloc dissection.<br />excision with intact capsule done <br /><ul><li>To prevent dissemination of contents,which cause a/c inflammatory response.
