  3. 3. <ul><li>“ Tethered” PHRs </li></ul><ul><li>PHR is integrated with or “tethered” to the provider’s EHR. </li></ul><ul><li>Information added/updated in PHR made available to the provider within their EHR. </li></ul><ul><li>PHR  EHR synchronization can occur automatically or manually. </li></ul><ul><li>Data exchange limited to the provider offering the PHR. </li></ul><ul><li>EHR is not a prerequisite. </li></ul><ul><ul><li>Data can transmit via fax from PHR to the provider for inclusion in record. </li></ul></ul>
