How to fill Passport Application Form

1. Visit Passport Seva Portal http://passportindia.gov.in
2. Register yourself as new user and login to your account.

1. Click the category as “Apply for Fresh Passport/Reissue of Passport”.
2. Click the highlighted link to fill the application form online.

Enter the Passport type information.

Enter applicant details & click “Save My Details” to avoid losing
entered data. Click on “Next” to proceed.

Enter family details.

Enter the present address details.

Enter the emergency contact details.

Enter references information.

Enter the Previous Passport details, if applicable.

Select options corresponding to other details.

Verify your details carefully

Select ‘I Agree’ checkbox and then click the ‘Submit Form’ button
to submit the form.

Make Payment Online / Offline
