This device should only be used under the continued supervision of a licensed physician. This
guide is not intended to replace or substitute the Intensity™ 5000 Hybrid Instruction Manual.
PLEASE READ THE INSTRUCTION MANUAL COMPLETELY BEFORE USING THIS DEVICE. Read,
understand and practice the warnings, cautions and operating instructions. Always follow the
operating instructions prescribed by your healthcare provider.

Steps 1 – 4 should be performed with the device turned OFF.

IMPORTANT! Once the unit has been turned ON, do not remove electrodes
without first turning OFF the device.

INSERT BATTERY
Press arrow and slide battery compartment
cover open. Insert 9V battery, matching the
+ and – ends to those in unit.

The included AC adapter may
also be used as a power source.

PLACE ELECTRODES
ON SKIN
Remove electrodes from
protective backing. Keep
backing to properly store
electrodes after use.

CONNECT ELECTRODES
TO LEAD WIRES

Turn the PW knob clockwise to select pulse width.
The pulse width is adjustable from 30 uS to 260 uS.

TURN DEVICE ON

	
Press the ON/OFF button located out the
bottom of the control panel. The mode LED indicator
and timer LED indicator will light up.

Turn the PR knob clockwise to select pulse rate.
The pulse rate is adjustable from 2 Hz to 150 Hz.

SELECT
THERAPY MODE

There are 5 therapeutic modes available:
B (burst), N (normal), M1 (pulse width
modulation), M2 (pulse rate modulation) and
SD (strength duration).

There are 4 therapy times available: 15 minutes, 30
minutes, 60 minutes and C for continuous.

This unit operates
with one or two sets
of electrodes. When
using one set, plug
lead wire into Channel
1 on top of the unit.
Use Channel 2 to
operate a second set
if directed to do so
by your healthcare
provider.

Make sure the bare metal of the pins is not exposed.

See Electrode Placement Guide for more information.

ADJUST PULSE WIDTH
AND PULSE RATE

CONNECT LEAD WIRES
TO TOP OF DEVICE

Firmly insert
the lead wire
connectors into
the electrode
connectors.

Apply electrodes to the
exact site indicated by
your doctor or therapist.
Make sure the skin at the
application site is clean
and dry.

TIMER

Press the TIMER button and the appropriate time LED
indicator will illuminate. Continue to press to select
desired time.

ADJUST CHANNEL
INTENSITY

Press the intensity control button
s and t to control the intensity
output. You will hear a “beep” sound.
Slowly press the intensity button
control until you reach the setting
recommended by your physician or
therapist. Repeat for other channel if
both channels are being used.
The Safety Lock Feature automatically locks the
settings if no changes have been made in the panel for
20 seconds. This feature prevents accidental changes to
your settings. Press the t button to unlock the device.

Once the unit has been turned ON, do not remove electrodes without first turning OFF the device. 1 INSERT BATTERY Press arrow and slide battery compartment cover open. Insert 9V battery, matching the + and – ends to those in unit. The included AC adapter may also be used as a power source. 4 PLACE ELECTRODES ON SKIN Remove electrodes from protective backing. Keep backing to properly store electrodes after use. 2 CONNECT ELECTRODES TO LEAD WIRES Turn the PW knob clockwise to select pulse width. The pulse width is adjustable from 30 uS to 260 uS. 5 TURN DEVICE ON Press the ON/OFF button located out the bottom of the control panel. The mode LED indicator and timer LED indicator will light up. Turn the PR knob clockwise to select pulse rate. The pulse rate is adjustable from 2 Hz to 150 Hz. SELECT THERAPY MODE There are 5 therapeutic modes available: B (burst), N (normal), M1 (pulse width modulation), M2 (pulse rate modulation) and SD (strength duration). MODE 8 9 SELECT THERAPY TIME There are 4 therapy times available: 15 minutes, 30 minutes, 60 minutes and C for continuous. N 15 M1 30 M2 60 N 15 M1 30 M2 60 SD C TIMER Press the MODE button and continue to press to select desired mode. ON/OFF MODE P.R. (Hz) 6 B B P.W. (uS) This unit operates with one or two sets of electrodes. When using one set, plug lead wire into Channel 1 on top of the unit. Use Channel 2 to operate a second set if directed to do so by your healthcare provider. Make sure the bare metal of the pins is not exposed. See Electrode Placement Guide for more information. ADJUST PULSE WIDTH AND PULSE RATE CONNECT LEAD WIRES TO TOP OF DEVICE Firmly insert the lead wire connectors into the electrode connectors. Apply electrodes to the exact site indicated by your doctor or therapist. Make sure the skin at the application site is clean and dry. 7 3 SD C TIMER Press the TIMER button and the appropriate time LED indicator will illuminate. Continue to press to select desired time. ADJUST CHANNEL INTENSITY Press the intensity control button s and t to control the intensity output. You will hear a “beep” sound. Slowly press the intensity button control until you reach the setting recommended by your physician or therapist. Repeat for other channel if both channels are being used. The Safety Lock Feature automatically locks the settings if no changes have been made in the panel for 20 seconds. This feature prevents accidental changes to your settings.
