Impact of LTC on Patients • Many appointments with many different HCP • HCPs tend to focus on guidelines • Lack of focus on patient’s priorities • Lack of continuity • Different advice from different professionals • Much medication and tests • Poor communication between GP and hospital • All time consuming and demoralising for patient
