  2. 2. *** NEW this year ***  Healthy Futures Program Participate and receive an incentive benefit in 2014-15  ODS changed name to MODA for medical insurance and added a plan  MODA uses letters (A-H) instead of numbers to identify plans  On-line enrollment is MANDATORY and REQUIRED by OEBB
  3. 3. Steps to Enroll 1. Go to 8/Default.aspx page to compare plans 2. Go to the calculator at e.aspx to calculate your monthly premium* 3. Log into to enroll in insurance plans and verify dependents 4. Revise as needed by September 8, 2013 * Rates will be different than the OEBB web site for Licensed and Classified staff due to an internal insurance pool.

The Benefits Team -- here to help!  By appointment for technical assistance with the online calculator or  Walk-ins OK at:  Century High School- 8/27, 10:30-7:30  Hillsboro High School- 8/28, 10:30-7:30  Liberty High School- 8/29, 10:30-7:30  Admin Center- LABOR DAY- 9/2, Noon-4:00  Benefits Fair- 9/5 1:30-6:30  Eligible staff member can enroll on their own any time or at one of these locations!
  5. 5. Why Enroll Early?  Technical ASSISTANCE if necessary  CORRECT plan elections and payroll deductions  OPTION to participate in the Flexible Spending Program (Pre-tax savings!)  PRIZE drawings – must enroll by September 8th (prizes provided by vendors)
