Medicaid is a state-federal program that provides essential services and benefits for low-income families and for many who have special health care needs. Military caregivers and their families may find themselves interacting with Medicaid in cases where TRICARE and ECHO coverage is not enough or when family members are not eligible for TRICARE and/or ECHO coverage. This year, there has been significant discussion about reforming and changing Medicaid in terms of the way it is funded, the services that are provided, and who may be eligible for the program. It is important for military support personnel, caregivers and military families to be aware of developments that are or might be underway in the Medicaid program.

  5. 5.  Current Trends and Realities  Origins and Evolution of Medicaid  Future Possibilities  Relevance to Military Families
  6. 6.  An insurance program that provides health coverage for many who are low income and/or have special health care needs.  Operated at the state level with federal and state funding.
  7. 7.  Provides basic services mandated by the federal government.  States may offer additional optional services.  Medicaid programs vary substantially from state to state.
  8. 8.  Eligibility and enrollment occurs at the state level – and is not transferable across state lines.  Eligibility is usually based on income.  Eligibility may also be based on health status.
  9. 9.  Serves individuals across all age groups  Most accessible for children, especially younger children.  An essential component for elder care, especially for long-term care.
  10. 10.  Medicaid is a major source of insurance in the United States.  Because of this, it shapes much of the American health care delivery system.  Primary and acute care services tend to be more accessible than specialty care.
  11. 11.  Under ACA, states have had the option to “expand” Medicaid to new adult low income populations.  Approximately half the states have done this, both blue and red.  Medicaid is a “budget buster” at the state level which creates fiscal strain and debate.
  12. 12.  The program has also gradually expanded health care coverage to Americans over its 50+ year history.  It influenced the development of the Children’s Health Insurance Program, now in its 20th year.
  13. 13. Over time, Medicaid has helped to shape new health care delivery approaches, including:  Managed care and medical homes  Child and maternal health  Home and Community Based Services.
  14. 14.  There is active discussion to scale back or repeal the ACA’s Medicaid expansion option  There is also active consideration of work or personal responsibility requirements for able-bodied Medicaid recipients  Medicaid has deeply entrenched and vested interests (states, hospitals, insurers) that will likely blunt the more radical reforms.
  15. 15.  The Medicaid program costs more than most states feel they can afford, so services and access may be limited.  Conflict and debate – balancing need and cost have always been part of the Medicaid picture
  16. 16.  Military families are not insulated from the civilian healthcare system.  Care for family members with special healthcare needs.  Care for older family members in long-term care situations.  Transition and separation planning
  17. 17.  Maximize services and resources offered by DHA (TRICARE, ECHO) and the military community.  Recognize that families may be “dually- eligible” for TRICARE and Medicaid.  Identify resources that provide assistance to families seeking to navigate Medicaid.
  18. 18.  Understand long-term care options under Medicaid and how they vary from state to state.  Recognize the importance of Home and Community Based Care approaches to service delivery.
  19. 19.  Anticipate the care demands of family members with special needs.  Consider economic situations on separation.  Think about long-term retirement planning
  20. 20. Medicaid is:  An insurance program for low-income and special needs populations.  Administered by states using federal and state funds.  A resource for all families, including active duty military families.
