  2. 2. What is a Form A form is a document (electronic or printed) with spaces for on which to write or enter data.
  3. 3. REASONS FOR FORMS Standardization – for collecting data in standard procedures. Ensure that all data needed is collected Data is sequenced in preferred priority
  4. 4. -cont They are handy sources of reference and can act as computer data input sources They ensure uniformity and consistency of approach. E.g, bank, insurance branches
  5. 5. Cont- Clerical staff can be readily trained to vet accurate completion of forms New forms can be readily designed to meet new or revised policies and procedures
  6. 6. Effective Form Design Take time to consider: Why is the info needed When it is needed Who needs it How info is best presented
  7. 7. Effective cont- Make a checklist of all the parts and pieces information needed Consider how the form might best be structured.
  8. 8. -cont Draw a mock-up of the form in rough Produce a test model of the form Print the form for use and check if the first users understand the form
  9. 9. Cont- Monitor your form from time to time to see if they are still relevant/ necessary.
