Paperwork Process PART 1 Due February 1, 2020  Personal Information Form (Online Form)  Passport information with picture of passport (Online Form/Upload)  Leadership survey (Pre-departure survey)  Consent and Release/Letter of Health Disclosure (Sign/Scan/Upload)  Health Form (Filled out by doctor/Upload)* - schedule appointment NOW!  Complete Amigos 101 Webinar  Single Parent Affidavit (Notarized/Upload) *Parent cannot fill out Health form for their child even if parent is a doctor.
  16. 16. HQ has committed to paying for a pizza party for all chapters who have 100% completion of paperwork requirements by the February 1st deadline!!!

Paperwork Process PART 2 Due April 15, 2020  Health Update (Online form available around Mar. 15)  Parent Permission to Travel Form - if under age 18** (Notarized/Upload)  Medical Power of Attorney- (Notarized/Upload)**  Background Check- Participants over age 18 **Participants must carry original permission to travel and medical POA with them in country.
  18. 18. HQ will charge an administrative fee of $250 to families who do not have ALL paperwork completed by May 1, 2020 as flight purchase will be delayed and group rate lost.

Understanding the Standards of Conduct  AMIGOS holds all volunteers to a set of high expectations outlined in our Standards of Conduct.  These allow us to partner with communities and agencies that would otherwise not collaborate with foreign youth.  These standards allow you to be safe within country.
