  1. 1. INFERTILITY OVERVIEWInfertility means difficulty in conceiving (becoming pregnant) despite havingregular sex without using contraception. There is no definite cut off point to saywhen a couple is “infertile’ many couples take several months to conceive. About17 in 20 couples conceive within a year of trying. About 19 in 20 couples conceivewithin two years.Looking at this another way, about 1 in 7 couples do not conceive within a year oftrying. However, more than half of these couples will conceive over the next yearwithout any treatment.

IUI (INTRA UTERINE INSEMINATION)• An intrauterine insemination (IUI) is a procedure in which the sperm is• “washed” and prepared then deposited into the upper portion of the uterus tohelp achieve a pregnancy. It is often in excellent option for couples prior toconsidering more complicated and very expensive procedures, such as in vitrofertilization(IVF).• The wash and IUI can be very helpful to couples seeking to have a child when theman has low sperm counts, low motility, hyper viscosity (very thick semen), motilesperm that are clumped together (antibodies), sperm that “ backs up” into thebladder (retrograde ejaculation) or other abnormal sperm test such a poor spermshape (morphology). Many couples use IUI to accelerate the chances of pregnancy,such as after vasectomy reversal or if the woman is older.• It is also useful when the woman’s mucus is hostile or compatible with man’ssperm, or if she has problems with her cervical positioning or opening, as well asunexplained causes of infertility.

The fundamental objective of the various preparation procedure employed isto separate the spermatozoa from the seminal plasma as thoroughly aspossible with minimum mechanical force. These are adequate for normalsemen sample. However, as the quality of reducing the level of physicaldamage inflicted upon the spermatozoa and at the separating thecontaminating material. The modifications fall into the following basiccategories. The basic aim of semen washing / processing is as follows:1. Removal of seminal plasma.2. Removal of non motile sperms, morphologically defective and immaturesperms.3. Removal of debris and pus cells.4. Concentration of motile sperms.5. Capacitating of motile sperms.

  11. 11. IVF (IN VITRO FERTILISATION) In vitro fertilisation (IVF) is a process by which an egg is fertilised by sperm outsidethe body: in vitro. IVF is a major treatment for infertility when other methods ofassisted reproductive technology have failed. The process involves monitoring awomans ovulatory process, removing ovum or ova (egg or eggs) from thewomans ovaries and letting sperm fertilise them in a fluid medium in a laboratory.When a womans natural cycle is monitored to collect a naturally selected ovum(egg) for fertilisation, it is known as natural cycle IVF. The fertilised egg (zygote) isthen transferred to the patients uterus with the intention of establishing asuccessful pregnancy. The first successful birth of a "test tube baby", Louise Brown,occurred in 1978. Louise Brown was born as a result of natural cycle IVF. Robert G.Edwards, the physiologist who developed the treatment, was awarded the NobelPrize in Physiology or Medicine in 2010.

ICSI (INTRA CYTOPLASMIC SPERM INJECTION)• ICSI is generally performed in addition an in vitro fertilization procedure to extractoften several oocytes from a woman.• The procedure is done under a microscope using multiple micromanipulationdevices (micromanipulator, microinjectors and micropipettes). A holding pipettestabilizes the mature oocyte with gentle suction applied by a microinjector. Fromthe opposite side a thin, hollow glass micropipette is used to collect a singlesperm, having immobilised it by cutting its tail with the point of the micropipette.The oocyte is pierced through the oolemma and into the inner part of the oocyte(cytoplasm). The sperm is then released into the oocyte.
