  • Just in case anyone needs to fill out CMS 1500 Insurance Claim Form, I found a blank form here http://goo.gl/aJVfjR. This site also has some tutorials on how to fill it out and a few related Insurance forms.

  • While some of the material is still relevant, the presentation needs to be updated. Also, many OCR vendors produce essentially the same raw character accuracy. Any minor differences are overwhelmed by the higher level business rules which are implemented for a solution -- to select alternate character values, auto-correct ambiguous values, and flag remaining suspicious or erroneous data.

    3. 3. New CMS 1500 Claim Form <ul><li>Reason for change: </li></ul><ul><li>The revised CMS 1500 (08/05) claim form accommodates the reporting of the National Provider Identifier (NPI) number. The NPI must be used by all HIPAA covered entities, such as health plans, health care clearinghouses, and health care providers. </li></ul><ul><li>No Barcode: </li></ul><ul><li>The barcode has been removed on the revised CMS1500 form. Scanners can be programmed to read the 1500 symbol in black ink. </li></ul><ul><li>Recommended transition timeline: </li></ul><ul><li>October 1, 2006: Health plans, clearing houses, and other information support vendor's should be ready to handle and accept the revised (08/05) 1500 Claim Form. </li></ul><ul><li>October 1, 2006 – March 31, 2007: Providers can use either the current (12/90) version or the revised (08/05) version of the 1500 Claim Form. </li></ul><ul><li>April 1, 2007: The current (12/90) version of the 1500 Claim Form is discontinued; only the revised (08/05) form is to be used. New UB04 Claim Form <ul><li>Why is it changing? </li></ul><ul><li>The UB04 contains a number of improvements and enhancements that include better alignment with the electronic HIPAA ASC X12N 837-Institutional Transaction Standard. The New UB04 paper billing form will be able to accommodate the reporting of the National Provider Identifier (NPI) Number. The NPI will be a single provider identifier, replacing the different provider identifiers healthcare systems currently use for each health plan with which you do business. The NPI Identifier, which implements a requirement of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), must be used by all HIPAA covered entities, which are health plans, healthcare clearinghouses, and healthcare providers. </li></ul><ul><li>What are the timelines for the transition of the new UB04? </li></ul><ul><li>March 1, 2007: Health plans, clearinghouses, and other information support vendors must be ready to handle and accept the new UB04 Billing Form. </li></ul><ul><li>March 1, 2007 – May 23, 2007: Providers can use either the current UB-92 or the new UB04 Billing Form. </li></ul><ul><li>May 23, 2007: The UB-92 is discontinued; only the new UB04 billing form is to be used. Benefits of Three Independent Ultrasonic Sensors Healthcare Insurance Form example Bob, Please review The left Ultrasonic sensor can be configured completely OFF or to ignore only specific areas of a document. In this case, set to ignore the area of the document that contains the image of the patient where clinic has taped the photo to the page. The middle Ultrasonic sensor is configured to detect multifeed documents. No multifeed detected on this example therefore scanning would continue normally. The right Ultrasonic sensor also has been configured to detect any multifeed documents. In this case a yellow sticky note is detected and the scanner can be setup to notify the operator and stop scanning and/or pause scanning in order to immediately take corrective action. Important information under the sticky note is not lost.

    11. 11. Optical Character Recognition NO. While it is legible to the human eye, the quality is too poor for use as an archival master file. The characters are too pale and uneven to OCR properly. Causes Poor Camera Technology / Inadequate Lamp Brightness NO. This scan is easier to read than the one above, but the characters have bled into one another and there is too much &quot;noise&quot;, or interference like the black marks surrounding the text,  to OCR the image. Causes Lack of Image Processing / Inefficient Image Enhancement YES. This scan is easily legible, looks like the original page, and will OCR without difficulty.
